Quel est le vécu de la lecture détaillée de la fiche de directives anticipées de la HAS par les médecins ?

Introduction - In 2016, the Claeys-Leonetti law made anticipated instructions opposable and the HAS proposed a drafting model. Despite this, they sparsely redacted. Method - In a double qualitative study, patients and doctors were asked about their experience of reading this model to understand why. Results - The sheet studied is perceived as a good support for discussion. However, participants say that the overly complex syntax and the lack of definition are inconvenient. They think that the model cannot be used by a patient alone because the options offered, too technical, do not allow him to make an informed choice. We also note that distrust is expressed by doctors and patients. It reflects a power issue between doctors, patients and the legal power, and is especially revealed when the subject of suffering is raised. Indeed, pain appears to be the first means by which we approach death nowadays. Nevertheless, the participants explained that conversing about death does not only imply the concept of suffering but concerns the patient as a whole. Thus, the fact sheet studied has the merit of offering a necessary dialogue around the end of life. The doctor is designed by patients as the main interlocutor to initiate this discussion between patient, relatives and doctor. The dialogue around the anticipated instructions makes it possible to begin to conceive the idea of death and to approach it more peacefully. Conclusion - The studied sheet is a way of initiating a necessary discussion regarding the end of life. Elles sont encore très peu rédigées malgré cela. Méthode : dans une double étude qualitative, patients et médecins ont été interrogés sur leur vécu de la lecture de ce modèle afin de comprendre pourquoi. Résultats : la fiche étudiée est perçue comme un bon support de discussion. Néanmoins, les participants disent que la syntaxe trop complexe, et le manque de définition sont des freins. Ils pensent que la fiche ne peut être utilisée par un patient seul car les choix proposés, trop techniques, ne lui permettent pas de s’exprimer de façon éclairée. On constate aussi une méfiance exprimée par les médecins et les patients. Cette dernière traduit un enjeux de pouvoir entre médecins, patients et le pouvoir juridique. On constate que celui-ci se révèle surtout lorsqu’on aborde la question de la souffrance. Cette dernière apparaît être le premier biais par lequel on aborde aujourd’hui sa mort. Néanmoins, les participants expliquent que parler de la mort n’implique pas que la notion de souffrance mais intéresse le patient dans sa globalité. La fiche étudiée a donc le mérite de proposer un dialogue nécessaire autour de la fin de vie. Le médecin est désigné par les patients comme intervenant principal pour initier ce dialogue entre patient, proches et médecin. Le dialogue autour des directives anticipées permet de se projeter sur sa mort et de l’aborder plus sereinement. Conclusion : la fiche étudiée est un moyen d’initier un dialogue nécessaire autour de la fin de vie. Ce dialogue, semble correspondre à une ébauche de rituel pour aborder la question de la mort
