Εndocrine and psychological changes during pregnancy and postpartum

The association between thyroid hormones and mood disorders in pregnancy and postpartum is not clearly established, regardless the considerable changes in the hypothalamic-pituitary thyroid (HPT) axis activity during these periods. This study aimed to examine potential associations between mood disorders and thyroid function, as well as serum cortisol, during pregnancy and postpartum in the same cohort of 93 pregnant women studied prospectively from 2015 to 2017. Participants underwent biochemical and psychometric tests at the 2nd and 3rd trimester of pregnancy and the 1st postpartum week. More specifically, serum thyroid hormones, TSH, anti-TPO, anti-Tg antibodies and cortisol measured and questionnaires- scales were employed at the same time-points to assess separately symptoms of anxiety (GADI, PSWQ, STAI-S, STAI-T), depression (EPDS, BLUES, BDI), obsessive compulsive disorder (Y-BOCS) and insomnia (AIS). At the 2nd trimester, GADI score correlated negatively with FT3 (p<0.010, r=-0.545) and positively with TSH (p<0.050, r=0.837) concentrations; GADI, PSWQ, EPDS and Y-BOCS scores correlated negatively with FT4 concentrations (p<0.010, r =-0.768; p<0.010, r =-0.384; p<0.050, r =-0.364; p<0.010, r=-0.544, respectively). PSWQ and Y-BOCS scores correlated positively with cortisol (p<0.050, r=0.345, p<0.050, r=0.426, respectively), while AIS score correlated positively with FT3 concentrations (p<0.050, r=0.344). At the 3rd trimester, BLUES score correlated positively with concentrations (p=0.00, r=0.89); GADI, EPDS, and Y-BOCS scores correlated negatively with FT4 concentrations (p=0.001, r=-0.468; p=0.036, r=-0.39; p=0.001, r=-0.625, respectively); GADI, STAI-S, and Y-BOCS scores correlated positively with TSH concentrations (p=0.015, r=0.435; p=0.024, r=0.409 p=0.041, r=0.389, respectively). At postpartum, PSWQ, STAI-T, EPDS, and BDI scores correlated positively with rT3 concentrations (p=0.024, r=0.478; p=0.014, r=0.527; p=0.046, r=0.44; p=0.021, r=0.556, respectively. BLUES score correlated positively with anti-TPO antibody concentrations (p=0.070, r=0.586), while Y-BOCS score correlated positively with TSH (p=0.045, r=0.43), and AIS score correlated positively with rT3 (r=0.431, p=0.045). The best predictor of postpartum BDI-score during the 2nd trimester was TSH (p=0.006, beta=-0.85), while in the 3rd trimester, cortisol was the best predictor of postpartum GADI-score (p=0.007, beta=0.709). Finally, in the 3rd trimester, TSH was the best negative predictor of Y-BOCS questionnaire postpartum (p= 0.022, beta=-0.792). In conclusion, the results of the present study demonstrate statistically significant associations of thyroid hormones and cortisol with psychometrics of mood disorders during pregnancy and postpartum. These findings indicate that thyroid hormone concentrations in mid- and late pregnancy and postpartum could be employed (alone or combined with other markers) to identify women at risk for developing mood disorders at pregnancy and postpartum.Η σχέση μεταξύ των θυρεοειδικών ορμονών και των διαταραχών διάθεσης στην κύηση και τη λοχεία δεν έχει πλήρως αποσαφηνιστεί, παρ’ όλο που ο υποθαλαμο- υποφυσεο- θυρεοειδικός (ΥΥΘ) άξονας παρουσιάζει αξιοσημείωτες μεταβολές κατά τις περιόδους αυτές. Σκοπό της παρούσας μελέτης αποτελεί η διερεύνηση της πιθανής συσχέτισης των διαταραχών της διάθεσης με τη θυρεοειδική λειτουργία και την κορτιζόλη κατά την περίοδο της κύησης και της λοχείας στην ίδια ομάδα 93 γυναικών, οι οποίες παρακολουθήθηκαν προοπτικά από το 2015 έως το 2017. Οι συμμετέχουσες υποβλήθηκαν σε βιοχημικό έλεγχο και ψυχομετρικές δοκιμασίες στο 2ο και 3ο τρίμηνο της κύησης καθώς και στην 1η εβδομάδα της λοχείας. Συγκεκριμένα, μετρήθηκαν στον ορό οι θυρεοειδικές ορμόνες, η ΤSH, τα anti-TPO και τα anti-TG, καθώς και η κορτιζόλη. Παράλληλα, επιλέχθηκαν ερωτηματολόγια- κλίμακες για την εκτίμηση των συμπτωμάτων του άγχους (GADI, PSWQ, STAI-S, STAI-T), της κατάθλιψης (EPDS, BLUES, BDI), της ιδεοψυχαναγκαστικής διαταραχής (Y-BOCS) και της αϋπνίας (AIS). Στο 2ο τρίμηνο της κύησης το σκορ της GADI συσχετίστηκε αρνητικά με τις συγκεντρώσεις της FT3 (p<0.010, r=-0.545) και θετικά με την TSH (p<0.050, r=0.837).
