Création d’un centre de consultations ambulatoires pendant la pandémie de Covid-19 : exemple du centre Covid-19 Paris 5. Etude du vécu des professionnels de santé quant à l’organisation et au fonctionnement du centre

Background: the Covid-19 pandemic in France has redesigned the health system organization. In the early stages of the outbreak, medical care of patients with suspected Covid-19 relied on the emergency services and hospital facilities, whereas in the third stage of the outbreak, it primarily relied on community medicine. Several ambulatory centers dedicated to Covid-19 have opened hroughout the national territory, as the Covid-19 Paris 5 center.Objective: the aim of this thesis was to evaluate the organization and the functioning of the center, and to identify its place in the management of the pandemic, through the practical experiences of health professionals who were working there.Method : a monocentric qualitative study was conducted. It relied on semi-structured interviews of health professionals who were working in the Covid-19 Paris 5 center. The phenomenological approach was followed for data analysis. Results: twenty-five health professionals were interviewed between May and June 2020. On the organizational level, the center was rapidly functional as a result of coordinate work and the setting up an office to take joint decisions. The recruitment of professionals was done on a voluntary basis. The center was accredited and funded by the Regional Health Authority. A shortage of individual protective equipment led to the modification of hygiene protocols to ensure health security. It took a while before the center became known among the population and the medical community of the arrondissement. The center had to evolve in accordance with government recommendations and the course of the epidemic, progressing from symptomatic patient management, to protecting the territory from a possible epidemic rebound, with screening of symptomatic patients and the search for epidemic clusters. Most health professionals valued their work at the center, and felt satisfied with their dedication and involvement in pandemic management. The center has enabled connections between professionals, which can lead to a better health network throughout the territory.Conclusion: the organization and the functioning of the center has optimized patient care and the development of measures to protect the territory against an epidemic rebound. An improvement in the spread of information to the population and the medical community would help the center to better integrate into the care pathway in times of health crisis.Contexte : la pandémie de Covid-19 en France a modifié l’organisation du système de soins. Alors qu’aux deux premiers stades, la prise en charge reposait sur le SAMU et les établissements hospitaliers, au niveau 3 du stade épidémique, la médecine de ville était en première ligne pour la prise en charge des patients suspects de Covid-19. Plusieurs centres de consultations ambulatoires dédiés à la Covid-19 ont alors ouvert sur l’ensemble du territoire, parmi lesquels le centre Covid-19 Paris 5, en mars 2020.Objectif : Cette thèse avait pour but d’évaluer l’organisation et le fonctionnement de ce centre, et d’identifier sa place dans la gestion de la pandémie, à travers le vécu des professionnels de santé y ayant travaillé.Méthode :une étude qualitative monocentrique par entretiens individuels semi-dirigés a été menée auprès des professionnels de santé ayant travaillé au centre Covid-19 Paris 5. L’analyse des données a été réalisée par l’approche hénoménologique.Résultats : vingt-cinq professionnels de santé ont été interrogés entre mai et juin 2020. Au niveau organisationnel, le centre a été rapidement fonctionnel grâce à un travail de coordination et à la mise en place d’un bureau pour une prise collégiale des décisions. Le recrutement des professionnels s’est fait sur la base du volontariat. Le centre a été accrédité et financé par l’ARS. Une pénurie de matériel de protection individuelle a amené à modifier les protocoles d’hygiène afin d’assurer une sécurité sanitaire. Le centre a mis du temps à se faire connaitre auprès de la population et de la communauté médicale de l’arrondissement. Il a dû évoluer selon les recommandations officielles et l’évolution épidémique, passant d’une prise en charge symptomatique des patients, à une protection du territoire contre une éventuelle reprise épidémique, avec un dépistage des patients symptomatiques et la recherche de foyers épidémiques. La majorité des professionnels de santé ont apprécié leur travail au centre, et se sont senti satisfaits de leur investissement et de leur contribution à la gestion de la pandémie. Le centre a permis la création de liens entre les professionnels, pouvant aboutir à un meilleur réseau de santé sur le territoire.Conclusion : l’organisation et le fonctionnement du centre ont permis une prise en charge optimisée des patients, et la mise en place de mesures de protection du territoire contre une reprise épidémique. Une amélioration de la diffusion d’informations auprès de la population et de la communauté médicale permettrait une meilleure insertion du centre dans le parcours de soins des patients en période de crise sanitaire
