Management Of Oral Ranula With Modified MicroMarsupialization: Case Report And Review Of Literature

Objective: Oral ranulas are cysts in the floor of the mouth that result from the extravasationof mucous. Historically there has been little consensus on the ideal first-line treatment, butcurrently, definitive treatment involves excision of the sublingual gland, which can injure thelingual nerve and submandibular duct. Minimally invasive surgical alternatives such as micromarsupialization have been proposed. This technique involves passing of 1 or more suturesthrough the base of the lesion to create alternate drainage pathway for the collected mucus sothat the cyst dissolves and dissipates by itself thereby avoiding surgery.Case: A 40-year-old female patient presented to our department with bubble-shaped lesionlocated on floor of the mouth, on the left side, with exophytic growth, sessile base, bluishcoloration measuring about 1 cm in diameter, showing well-defined limits and borders, smoothsurface, and softened consistency. Under the diagnostic hypothesis of the ranula, a modifiedmicro-marsupialization was performed. After the 50 days of observation, complete regressionand repair of the lesion was observed without recurrence.Conclusion: Modified micro-marsupialization is a simple, minimally invasive, low-cost,comfortable technique for both patient and operator with a decent percentage of successiverate. It is well-tolerated by the patient, mainly uncooperative patient.
