Effect of gluten-free casein-free diet on gastrointestinal symptoms in children with autism spectrum disorder

Otizm spektrum bozukluğu (OSB) olan çocuklar, beslenme sorunlarını ve gastrointestinal sistem semptomlarını normal gelişmekte olan çocuklara göre sık yaşamaktadır. Davranışsal ve gastroinstestinal semptomları (GİS) iyileştirmek amacıyla uygulanan beslenme müdahalelerinden biri glutensiz-kazeinsiz diyet (GFCF)'tir. Bu çalışmanın amacı; OSB'li çocuklarda GFCF diyetinin GİS semptomlarına etkisini incelemektir. Çalışmaya 27 gönüllü OSB'li çocuk katılmış, ebeveyn ve çocuğun tanımlayıcı özellikleri, beslenme durumu sorgulanmış, GİS semptomları izlem formu, Otizm Öğün Davranış Kısa Ölçeği (BAMBI), Otizmde Yaşam Kalitesi Anketi-Ebeveyn Sürümü (OYKA-E) uygulanmıştır. Uygulanan formlar çalışma sonrası tekrarlanmıştır. GFCF diyet (n=12), kontrol grubu olarak (n=15) ayrılan katılımcıların yaş ortalaması 6,50±2,39, 5,33±1,71 yıldır ve %36,3'ü obez, %33,3'ü hafif şişmandır. GFCF diyet grubunun %58,3'ü, kontrol grubunun %66,7'si GİS semptomlarının bir veya birkaçını yaşamakta ve diyet sonrası konstipasyon, karın ağrısı, gaz, dışkıda sindirilmemiş yiyecek parçaları görülen OSB'li çocuk sayısı azalmıştır. Karın ağrısı, konstipasyon, gaz görülen çocuk sayısı sırasıyla 7'den 3'e, 8'den 3'e, 6'dan 3'e düşmüş ve dışkıda sindirilmemiş yiyecek parçaları görülen 5 çocuğun hiçbirinde bu sorun görülmemiştir. Azalan GİS semptomlarının öğün davranışı ve beslenme sorunlarını etkilemediği belirlenmiştir. GFCF diyeti sonrası OSB'li çocukların öğün davranışı ve beslenme sorunlarını daha az yaşadığı bulunmuştur. GFCF diyet grubunun diyet sonrası karbonhidrat ve kalsiyum alımları istatistiksel olarak anlamlı şekilde azalmıştır (p0,05). GFCF diyetinin GİS semptomlarını iyileştirici etkisi olabileceği, ebeveynlerin diyetisyen danışmanlığında çocukların büyüme ve gelişmesi izlenerek, gerekli besin destekleri yapılarak diyeti sürdürmeleri gerektiği sonucuna varılmıştır.Children with autism spectrum disorder (ASD) experience feeding problems and gastrointestinal system symptoms more frequently than children with normal development. One of the nutritional interventions applied to improve behavioral and gastrointestinal symptoms (GIS) is gluten-free-casein-free diet (GFCF). The aim of this study; To investigate the effect of GFCF diet on GIS symptoms in children with ASD. İn the study, 27 volunteer children with ASD participated, descriptive characteristics of parents and children, nutritional status were questioned, GIS symptoms follow-up form, Autism Meal Behavior Brief (BAMBI), Autism Quality of Life Questionnaire-Parent Version (OYKA-E) were applied. The applied form was repeated after the study. The mean age of the participants on the GFCF diet (n=12), control group (n=15) is 6.50±2.39, 5.33±1.71 years and 36.3% are obese, 33.3% slightly fat. Of the GFCF diet group 58.3% and the control group 66.7% of experience one or more of the GIS symptoms, and childeren with ASD post-diet constipation, abdominal pain, gas, and the number of food particles in the stool decreased. The number of children with abdominal pain, constipation, and gas decreased respectively from 7 to 3, 8 to 3, 6 to 3 and none of the 5 children with food particles in the stool had this problem. It was determined that decreased GIS symptoms did not affect meal behavior and nutritional problems. Children with ASD after the GFCF diet Meal behavior and feeding problems are less found. Post-diet carbohydrate and calcium intakes of the GFCF diet group were statistically significantly decreased (p0.05). It was concluded that the GFCF diet may have a healing effect on GIS symptoms, and the diet should be maintained by monitoring the growth and development of the children under the dietitian consultation of the parents, and providing the necessary nutritional support
