Everyday life of adolescents with sickle cell disease – A qualitative, explorative and participative photovoice study

Hintergrund: Die Sichelzellkrankheit (Sickle Cell Disease, SCD) ist eine in der Schweiz seltene, chronische, hämatologische Erbkrankheit. Sie löst unter anderem wiederholt heftige Schmerzkrisen aus, ruft schwere Infektionen hervor und erfordert häufige Hospitalisationen. Gegenwärtig ist wenig bekannt, was aus der Perspektive von Jugendlichen die Alltags- erfahrungen mit SCD sind. Ziel: Mit der Studie wurde der Frage nachgegangen was den Alltag von Jugendlichen mit SCD prägt und welche Herausforderungen sich zeigen. Daraus sollten entsprechende Unterstützungsangebote für das Pädiatrische Zent- rum für Hämoglobinopathie abgeleitet werden. Methoden: Die qualitative Studie erfolgte mittels der partizipativen Methode Photovoice. Diese umfasst, nebst der Arbeit mit Fotos, eine Gruppendiskussion und eine rekonstruktive Analyse. Ergebnisse: Die Jugendlichen mit SCD beschrieben Alltagsthemen in den Dimensionen Lebenswelt (z. B. Familie), Bewältigungsstrategien (z. B. Peers) und Gesundheitskompetenz (z. B. Krankheitsverständnis), die zugleich Herausforderungen und Ressourcen sind. Daraus konnten die Themen Stärkung des Selbstmanagements und Gestaltung der Leistungserbringung als Grundlage für ein Betreu- ungs- und Behandlungskonzept identifiziert werden. Schlussfolgerungen: Jugendliche mit SCD zeigen in vielen Bereichen ihres Lebens Anzeichen von Gesundheitskompetenz. Hier kann die Angebotsentwicklung des Pädiatrischen Zentrums ansetzen. Erfolgsversprechende Methoden dafür sind Partizipation und der Einbezug von Peers in Beratung und Forschung. The disease is characterized by recurring attacks of pain and serious infections that require frequent hospitalisation. Due to its rarity, little is known about the disease's impact on the daily lives of afflicted adolescents. Aim: The study investigated how the disease impacts on the lives of adolescents with SCD and identified the most pertinent challenges they face in their daily activities. Knowledge gained serves to improve support offered by the Paediatric Haematological Centre. Method: Quali- tative data was collected using the participative method Photovoice. This involved an integrated group discussion of photo- graphs taken by the participants, and was followed by reconstructive analysis. Results: Issues voiced covered living environ- ment (e. g. family), coping strategy (e. g. peers) and health literacy (e. g. understanding their condition). These issues were perceived as challenges as well as a resource. Self-management and Shaping of health management emerged as key issues for a support and care concept in nursing and health counselling of SCD. Conclusions: Adolescents with SCD exhibited health literacy in various areas of their lives that can be fostered by the health care system.
