Correlación clínica, imagenológica e histopatológica en tumores malignos de la glándula mamaria. Experiencia en el hospital Dr. Francisco Antonio Risquez (2004-2014)

Introduction: Breast cancer represents the first cause of death in women in our country and is the pathology that is diagnosed more frequently in women worldwide. Objective: To determine the clinical, imaging and histopathological correlation of malignant tumors of the mammary gland in patients surgically treated in the General Surgery Department of the Dr. Francisco Antonio Risquez Hospital in the period 2004-2014. Methods: A retrospective, cross-sectional epidemiological study was conducted. All the patients who underwent surgery in our department for malignant tumors of the mammary gland, who had a complete clinical history, were included. Results: 47.83 % were patients of 41-50 years, most consulted for presenting palpable mass. Tumors were more frequent in the right breast, measuring 52.17 % >≥2 and <5 cm, being mobile in 65.21 % of the cases and non-painful in 30.43 %. The 52.17 % of mammograms were BI-RADS 0, only 5 cases BI-RADS 4 and 2 cases BI-RADS 5. Most of the ecosonograms reported mammary tumor. It was observed that 52.17 % of PAAF (Puncture with Fine Needle Aspiration), reported ductal carcinoma. Partial mastectomy and modified radical were used as therapeutic options. The 92.30 % of the cases turned out to be ductal carcinoma in the definitive biopsy. There is an association between PAAF, the ecosonogram, mammography and definitive biopsy, with mammography being the weakest relation, in this study the sensitivity of PAAF was 86.96 %, that of the ecosonogram 82.61 % and mammography 30.43 % The specificity for the three tests was 100 %. Conclusions: For the diagnosis of malignant tumors of the mammary gland, it is fundamental to correlate the clinical, imaging and histopathological studies and in this way increase their sensitivity and specificity, making detections in early stages, improving prognosis and morbidity.-mortality.Introducción: El cáncer de mama representa la primera causa de muerte en mujeres en nuestro país y es la patología que se diagnostica con mayor frecuencia en las mujeres a escala mundial. Objetivo: Determinar la correlación clínica, imagenológica e histopatológica de los tumores malignos de la glándula mamaria, en pacientes intervenidas quirúrgicamente en el servicio de Cirugía General del Hospital Dr. Francisco Antonio Risquez en el periodo 2004-2014. Métodos: Se realizó un estudio epidemiológico, retrospectivo, de corte transversal. Se incluyeron todas las pacientes intervenidas quirúrgicamente en nuestro servicio por tumores malignos de la glándula mamaria, que contaran con historia clínica completa. Resultados: El  47,83 % eran pacientes de 41-50 años, la mayoría consultó por presentar masa palpable. Los tumores se presentaron con mayor frecuencia en la mama derecha, midiendo en el 52,17 % ≥> 2 y < 5 cms, siendo móviles en el 65,21 % de los casos y no dolorosos en el 30,43 %. El 52,17 % de las mamografías fueron BI-RADS 0, solo 5 casos BI-RADS 4 y 2 casos BI-RADS 5. La mayoría de los ecosonogramas reportaron tumor mamario. Se observó que el 52,17 % de las PAAF (Punción con Aspiración con Aguja Fina), reportaron carcinoma ductal. Se utilizaron la mastectomía parcial y la radical modificada como opciones terapéuticas. El 92,30 % de los casos resultaron ser carcinoma ductal en la biopsia definitiva. Existe asociación entre la PAAF el ecosonograma, la mamografía y la biopsia definitiva, siendo la mamografía la relación más débil, en este estudio la sensibilidad de la PAAF fue de 86,96 %, la del ecosonograma 82,61 % y la mamografía 30,43 %. La especificidad para las tres pruebas fue del 100 %. Conclusiones: Para el diagnóstico de los tumores malignos de la glándula mamaria, es fundamental correlacionar la clínica, los estudios imagenológicos e histopatológicos y de esta manera aumentar la sensibilidad y especificidad de los mismos, realizando detecciones en estadios tempranos, mejorando el pronóstico y la morbi-mortalidad
