Congenital syphilis: a misleading disease

La sífilis es una enfermedad producida por una bacteria de tipo espiroqueta Treponema Pallidum que puede ser transmitida por vía sexual y maternofetal, adquirida y congénita respectivamente. Su afección es sistémica, de evolución crónica y en ocasiones asintomática. La probabilidad de la infección congénita va a depender del estadio en que se encuentre la progenitora y el momento de la aparición. La sífilis durante el embarazo, produce riesgo tanto para la madre como para el niño, por eso el tratamiento debe ser para ambos: resolver el proceso infeccioso de la gestante y prevenir la infección intrauterina del feto. Existen factores de riesgo que pueden producir un aumento de casos de esta enfermedad de transmisión sexual tales como, pobre educación en salud reproductiva, control de salud prenatal deficiente, falta de acceso a pruebas de laboratorio, relaciones sexuales de riesgo o múltiples parejas, drogas intravenosas, trabajadoras sexuales y antecedente de sifilis en embarazos previos o de enfermedades de transmisión sexual. La sífilis congénita es una enfermedad controversial, ya que más de un 50% de los niños al nacer que la padecen son asintomáticos y su clínica suele parecer durante los primeros tres meses de vida. Se puede dividir en dos síndromes típicos: la congénita temprana y la congénita tardía. Es fundamental la captación temprana de la sífilis en el embarazo, idealmente en la primera cita del control prenatal. El tamizaje se debe repetir entre las semanas 28 y 32 de embarazo y durante el parto en mujeres con alto riesgo. El estudio más utilizado es el VDRL, la historia clínica sexual y examen físico. La prueba confirmatoria son las pruebas treponémicas convencionales (FTA-ABS y TPPA). La bencilpenicilina benzatina es el único medicamento efectivo y seguro en el embarazo y en el feto, ya que atraviesa la barrera placentaria.Sexually transmitted diseases, mainly syphilis, are a public health problem. Congenital syphilis is a controversial disease, since more than 50% of children who suffer from it at birth are asymptomatic and its symptoms usually appear during the first three months of life. It can be divided into two typical syndromes: early congenital (in the first two years of life) and late congenital (after 2 years). Early detection of syphilis in pregnancy is essential, ideally at the first prenatal appointment. Screening should be repeated between weeks 28 and 32 of pregnancy and during birth of high risk. The most used test is the VDRL, sexual clinical history and physical examination and the confirmation is the conventional treponemal tests (FTA-ABS and TPPA). Benzathine benzylpenicillin is the only effective and safe drug to use during pregnancy and to the fetus, since it crosses the placental barrier
