Terapia comportamental associada a psicofarmacoterapia em pacientes com transtorno obsessivo-compulsivo: um estudo aberto

OBJECTIVE: The authors carried out a prospective open study by following-up on 20 patients diagnosed with obsessive-compulsive disorder for 6 months. Patients were treated with association of behavior therapy and pharmaceuticals which inhibit serotonin reuptake. The objective of the study was to verify the efficacy of behavior therapy and psychopharmacotherapy association in reducing obsessive-compulsive symptoms.MATERIALS AND METHODS: In order to evaluate treatment results we used the Yale-Brown Obsessive-Compulsive Scale (YBOCS), the National Institute of Mental Health Global Scale (NIMH), and the Subjective Units of Discomfort Scale (SUDS) before the treatment and at 3 and 6 months of treatment. RESULTS: We observed a medial reduction of symptom intensity at 3 months (42% in the YBOCS; 14.28% in the NIMH scale; and 24.4% in the SUDS) and at 6 months(72% in the YBOCS; 71.42% in the NIMH scale; and 61.9% in the SUDS). The combined treatment was effective in 75% of patients at 3 months, and in 85.6% of patients at 6 months.CONCLUSIONS: Results were independent from age, schooling, marital status, duration of disorder, how disorder started, presence or not of precipitating event, and intensity of symptoms at the beginning of the treatment. However, patients which had previously used reuptake inhibitors, and were evaluated by the YBOCS scale at 6 months, presented a reduction of symptom intensity that was significantly smaller (P< 0.0421).OBJETIVO: Os autores realizaram um estudo aberto prospectivo, acompanhando durante 6 meses, 20 pacientes com diagnóstico de transtorno obsessivo-compulsivo, tratados com a associação de terapia comportamental (TC) e fármacos inibidores da recaptação (IRS) da serotonina com o objetivo de verificar a eficácia da associação de terapia comportamental e psicofarmacoterapia na redução de sintomas obsessivocompulsivos. MATERIAIS E MÉTODOS: Foram utilizados como instrumentos para avaliar os resultados do tratamento as escalas YBOCS, Escala Global do NIMH, e EscalaSubjetiva de Grau de Desconforto (SUDS), aplicadas antes do início, aos 3 e aos 6 meses de tratamento.RESULTADOS: Foi observada uma redução média na intensidade dos sintomas, aos 3 meses de 42% na escala YBOCS, de 14,28% na escala NIMH e de 24,4% naescala SUDS; aos 6 meses a redução foi de 72% na escala YBOCS, 71,42% na escala NIMH e de 61,9% na escala SUDS, respectivamente. O tratamento combinadose mostrou eficaz em 75% dos pacientes aos 3 meses, e em 85.6% aos 6 meses.CONCLUSÕES: Os resultados se revelaram independentes da idade, nível de instrução, situação conjugal, tempo de duração do transtorno, modo de início, presença ou não de evento precipitante, e da intensidade dos sintomas no início do tratamento. Entretanto pacientes que anteriormente haviam utilizado IRS obtiveram uma redução menor na intensidade do sintomas, avaliados pela escala YBOCS aos 6 meses, num nível estatisticamente significativo, (p<0.0421)
