Two-year evaluation of a multifamily psychoeducational program (PROTEC) in the family burden and prognosis of bipolar patients

Introduction Bipolar disorder causes a significant burden on the lives of patients and their families. The family is one of the targets for therapeutic intervention, related to the prognosis in patients with bipolar disorder. Aim To assess the effectiveness of a multifamily psychoeducational program for people with bipolar disorder, in the family burden: objective and subjective and in the variables related to the course of the patients with bipolar disorder (symptoms, adherence, functionality, hospitalizations), comparing it with a control group (CG). Materials and methods A total of 148 relatives of bipolar patients and 148 bipolar patients were recruited. The sample was randomized (experimental group [EG] and CG) and with single-blind evaluations (baseline, at 5 months and one year). Clinical and sociodemographic variables were collected from families and patients (family burden self-report scale, Strauss-Carpenter Scale, Global Assessment of Functioning, Morisky Green adherence Scale). Both, EG and CG received 8 multifamily sessions, applied exclusively on the relatives of patients with bipolar disorder, but in the EG a psychoeducational treatment was carried out and in the CG only playful and current topics were discussed. Bivariate and logistic regression models were used, among others. Results The caregivers and patients of the EG and CG did not differ in any of the baseline variables (sociodemographic and clinical) (P > .001). In the total sample, the baseline objective burden was light (mean 0.6 ± 0.4) and the subjective ones was medium-moderate (mean 1.1 ± 0.3). During the follow-up, in relation to the variables of the caregivers, there was a greater reduction in the objective burden in the EG compared to the CG (5 months P = .006; one year P = .002). It was found that the objective burden (P = .006) and the subjective burden (P = .003) were significantly reduced over a year in EG but not in the CG. During the follow-up, the patients whose caregivers belonged to the EG showed a greater increase in the frequency of social activity (P = .008), in the work activity (P = .002), and global functioning (P = .002), and reduced their symptoms (P ≤ .001). Longitudinal analyses, over a year, showed that patients in the EG had a greater improvement in functionality compared to patients in the CG (P = .001). After the intervention, adherence to pharmacological treatment improved more in EG than in the CG (P ≤ .001). Regarding hospitalizations, any patients in the CG were hospitalized during the 5 months after the intervention, while 27.8% of the patients in the CG were hospitalized (P ≤ .001); the difference between groups remained significant in the long term (one year: P ≤ .001; 2 years: P ≤ .001). There were no significant differences between groups in the pharmacological treatment of the patients in any of the evaluations. Conclusions The multifamily psychoeducational intervention group improved the family burden after the intervention. Likewise, bipolar patients, whose families attended the EG, improved significantly, over a year, the functionality, the frequency of social contacts, the work status, the adherence to treatment, and reduced their symptoms. In addition, in the EG, the percentage of hospitalizations during the 2 years of follow-up was significantly reduced.Introducción El trastorno bipolar produce una carga importante en la vida de los pacientes y sus familiares. La familia es una de las dianas para la intervención terapéutica, que se ha encontrado que se relaciona con el pronóstico de los pacientes con trastorno bipolar. Objetivo Valorar la efectividad de un programa psicoeducativo multifamiliar de personas con trastorno bipolar en la carga familiar: objetiva y subjetiva y en las variables relacionadas con el curso de los pacientes con trastorno bipolar (síntomas, adherencia, funcionalidad, hospitalizaciones), comparándolo con un grupo control (GC). Materiales y métodos Se seleccionaron 148 familiares de pacientes bipolares y 148 pacientes bipolares. La muestra fue aleatorizada (grupo experimental [GE] y GC) con evaluación enmascarada (basal, 5 meses y anual). Se recogieron variables clínicas y sociodemográficas de familias y pacientes (escala de autoinforme de sobrecarga familiar, escala de Strauss-Carpenter, escala de evaluación del funcionamiento global, escala de adherencia Morisky-Green). Tanto el GE como el GC recibieron 8 sesiones multifamiliares, aplicadas exclusivamente sobre los familiares de pacientes con trastorno bipolar, pero en el GE se realizó un tratamiento psicoeducativo y en el GC únicamente se trataron temas lúdicos y de actualidad. Se utilizaron, entre otros, modelos de regresión bivariada y logística. Resultados Los cuidadores y pacientes del GE y del GC no difirieron en ninguna de las variables basales (sociodemográficas y clínicas) (p > 0,001). En toda la muestra, la carga objetiva basal fue leve (media 0,6 ± 0,4) y la subjetiva fue media-moderada (media 1,1 ± 0,3). Durante el seguimiento, en relación con las variables familiares, hubo una mayor reducción de la carga objetiva en el GE respecto al GC (5 meses: p = 0,006; un año: p = 0,002). Se encontró que en el GE, pero no en el GC, al año, se redujo significativamente la carga objetiva (p = 0,006) y la carga subjetiva (p = 0,003). Durante el seguimiento, los pacientes cuyos familiares pertenecían al GE mostraron una mejora de la frecuencia de actividad social (p = 0,008), de la situación laboral (p = 0,002) y del funcionamiento global (p = 0,002), y una reducción de su sintomatología (p ≤ 0,001). Los análisis longitudinales a lo largo de un año mostraron que los pacientes en el GE tenían una mayor mejora en la funcionalidad en comparación con los pacientes en el GC (p = 0,001). Tras la intervención, la adherencia al tratamiento farmacológico mejoró más en el GE que en el GC (p ≤ 0,001). En cuanto a las hospitalizaciones, en la evaluación a los 5 meses, el 0% de los pacientes del GE fueron hospitalizado frente al 27,8% de los pacientes en el GC (p ≤ 0,001); esta diferencia entre grupos siguió siendo significativa a largo plazo (un año: p ≤ 0,001; 2 años: p ≤ 0,001). No hubo diferencias significativas entre grupos en el tratamiento farmacológico de los pacientes en ninguno de los puntos de evaluación. Conclusiones Se encontró una mejora significativa en el grupo de intervención psicoeducativa multifamiliar en la carga familiar, objetiva y subjetiva, después de la intervención. Asimismo, los pacientes con trastorno bipolar cuyas familias formaban parte del GE mejoraron la funcionalidad, la frecuencia de los contactos sociales, la situación laboral, la adherencia al tratamiento farmacológico y la sintomatología a lo largo de un año. Además, el porcentaje de hospitalizaciones del GE a lo largo de los 2 años de seguimiento se redujo significativamente
