Ictère du nouveau-né et sortie de maternité. Un bilan en Ile-de-France 2012

Neonatal jaundice and the exit of maternity hospital. Assessment in Ile-de-France 2012. Neonatal jaundice is a frequent pathology which concerns 60 % of the newborn. Among these neonatal jaundice, 2,5 in 5,0% will develop into a severe hyperbilirubinemia. Since a few years, we have noted an outbreak of the number of cases of kernicterus. Faced with this report, we tried - thanks to two retrospective studies led between July, 2012 and January, 2013 with the paediatricians of Ile-de-France and the parents coming to consult in the National Reference center in Hémobiologie Périnatale - to study the organization of the system of screening, diagnosis and care as well as to identify its failures. The results of this study show that non-consensual pratices persist in these maternity hospitals and reveal the dissatisfaction of a third of the relatives with the information received. In the light of these results, progress regarding screening, diagnosis and care seem necessary, together with an improvement of the quality and the effenciency of the information given to the relatives.L'ictère néonatal est une pathologie fréquente puisqu'il concerne 60% des nouveau-nés. Parmi ces ictères, 2,5 à 5,0% évolueront vers une hyperbilirubinémie sévère. Depuis quelques années, on assiste à une recrudescence du nombre de cas d'ictère nucléaire. Devant ce constat, nous avons cherché - grâce à deux études rétrospectives menées entre juillet 2012 et janvier 2013 auprès des pédiatres d'Ile-de-France et des parents venant consulter au Centre National de Référence en Hémobiologie Périnatale - à étudier l'organisation du système de dépistage, de diagnostic et de prise en charge ainsi qu'à identifier ses défaillances. Les résultats de ces enquêtes montrent que des pratiques non consensuelles persistent dans ces maternités et mettent en relief l'insatisfaction d'un tiers des parents interrogés pour l'information qu'ils ont reçue. Au vu de ces résultats, des progrès en matière de dépistage, de diagnostic et de prise en charge sembleraient nécessaires ainsi qu'une amélioration de la qualité et de l'efficience de l'information délivrée aux parents
