Public health as a new clinical approach to oral cancer screening and diagnosis

Malgré les progrès thérapeutiques, les taux d’incidence et de survie à 5 ans liés aux cancers oraux ne se sont pas améliorés et demeurent un problème majeur de santé publique. La guérison et la survie des patients étant directement liées au stade de développement de la tumeur au moment du diagnostic, l’objectif de ce travail était d’analyser l’ensemble des déterminants liés au cancer oral et de proposer de nouvelles approches cliniques de diagnostic et de dépistage. La première partie de ce travail a été consacrée à l’identification des facteurs épidémiologiques et biologiques utiles à la compréhension de la cancérogénèse orale et des stratégies envisageables pour en améliorer les taux d’incidence et de mortalité associés.La seconde partie de ce travail a permis de constater l’existence d’un défaut de formation et de connaissances des professionnels de santé. Une analyse infodémiologique portant sur les cancers oraux dans l’Union Européenne a permis de proposer 8 axes de ré-articulation des campagnes nationales de sensibilisation. La troisième partie de ce travail était destinée à évaluer l’intérêt de la mise en place d’un nouvel outil d’aide au diagnostic et pourquoi aucun consensus n’existe encore au sein de la communauté scientifique.Une étude préclinique a été conduite afin d’identifier l’apport de la microscopie multiphotonique dans le diagnostic précoce de cette maladie. Sont proposées, en conclusion de ce travail, la mise en place de nouveaux moyens de formations ainsi que des perspectives d’organisation de campagnes de sensibilisation et de dépistage du cancer oral spécifiquement destinées aux population à risque.Despite therapeutic advances, the incidence and 5-year survival rates of oral cancer have not improved and remain a major public health problem. 70% are still diagnosed at a late stage, with an average delay of 2-5 months. As the cure and survival of patients are directly related to the stage of development of the tumour at the time of diagnosis, the aim of this work was to analyse all the determinants related to oral cancer and to propose new clinical approaches to diagnosis and screening. The first part of this work was devoted to the identification of epidemiological and biological factors useful for the understanding of oral carcinogenesis and possible strategies to improve the associated incidence and mortality rates, based on a critical reading of the literature.The second part of this work allowed, within the framework of an evaluation of professional practices, to note the existence of a lack of training and knowledge of health professionals. An infodemiological analysis of oral cancers in the European Union made it possible to propose 8 axes for the re-articulation of national awareness campaigns. The third part of this work was intended to evaluate the interest of setting up a new diagnostic aid tool and why no consensus yet exists within the scientific community.A preclinical study was conducted to identify the contribution of multiphoton microscopy in the early diagnosis of this disease. The conclusion of this work is to propose the implementation of new training methods as well as the organisation of awareness and screening campaigns for oral cancer specifically aimed at the population at risk
