Observance non médicamenteuse des patients atteints d'hypertension artérielle et influence de la relation qu'ils entretiennent avec leur médecin traitant

Context: One of the main causes of insufficient blood pressure control is poor compliance with dietary health measures (DHM) by hypertensive patients. However, the majority of hypertensive patients are followed by general practitioners (GPs). Objective: To explore and understand the relationship between adherence to DHMs by patients with controlled and uncontrolled arterial hypertension (AHT) and their relationship to their GP. Method: Qualitative exploratory study using a phenomenological approach, based on 10 semi-structured interviews with patients followed for AHT by GPs in a health center in the Île-de-France region. Coding and thematic analysis were performed by 3 researchers. Results: AHT is perceived as a benign disease, the cause of which is not related to poor health behaviors. There is a good relationship between the patient and their GPs in the context of AHT. Patients describe their need for patient-centered medicine, with more transparency about the disease and personalized support to promote better compliance with DHM. The patient must be considered as a whole in order to obtain adherence to DHM. Tobacco appears to be ignored and medication takes a center place at the expense of DHM. A time of exchange with the GP and a motivational approach seem to be useful levers to induce a change in behavior. Conclusion: Improving adherence to DHD in the context of AHT requires taking into account the patient as a whole.Contexte : Une des causes principales d’un contrôle tensionnel insuffisant est la mauvaise observance des mesures hygiéno-diététique (MHD) par les patients hypertendus. Or la majorité des patients hypertendus sont suivis par des médecins généralistes (MG). Objectif : Explorer et comprendre la relation entre l’observance des MHD des patients ayant une hypertension artérielle (HTA) contrôlée ou non, et leur relation avec leur MG. Méthode : Étude qualitative exploratoire selon une approche phénoménologique, à partir de 10 entretiens semi-dirigés avec des patients suivis pour l’HTA par des MG en maison de santé dans la région Île-de-France. Le codage et l’analyse thématique ont été réalisés par 3 chercheurs. Résultats : L’HTA est vécue comme une maladie bénigne, dont la cause n’est pas liée aux mauvais comportements de santé. Il existe une relation de qualité entre le patient et son MG dans le cadre de l’HTA. Les patients décrivent leur besoin d’une médecine centrée patient, avec plus de transparence sur la maladie et un accompagnement personnalisé pour favoriser une meilleure observance des MHD. Il faut prendre en compte le patient dans sa globalité pour obtenir son adhésion au MHD. Le tabac semble être ignoré et les médicaments prennent une place centrale au dépend des MHD. Un temps d’échange avec le MG et une approche motivationnelle semblent être des leviers utiles pour induire un changement de comportement. Conclusion : Améliorer l’observance des MHD dans le cadre de l’HTA nécessite de prendre en compte le patient dans sa globalité, dans un climat de confiance, avec une communication adaptée, pour une meilleure promotion des MHD par le MG
