La relation médecin-patient dans la construction identitaire de patients trans : étude qualitative auprès de patients et leurs médecins généralistes

General practitioners (GP) will be increasingly confronted with trans individuals, because the trans population increases. The aim of this work was to understand the experiences of general practitioners and their trans patients towards gender identity building. METHODS: This work is a qualitative research on patient-GP pairs. 11 one-on-one semi-structured interviews took place between August 2020 and April 2021. A phenomenological analysis was conducted. RESULTS: Seeking care was difficult for interviewed patients, due to a mistrust in the physicians. The patients chose carefully and tested their GP. They were expecting “classical” care, with a comprehensive approach. The vision of GP over the transition was biologically and medically centered. They didn’t raise the importance of social transition, which was described by the patients. The patients explained that they were continuously questioning their identity, whereas the GP thought that the questioning took place exclusively in the past. The patients ensured the coordination of their healthcare circuit, thanks to their expertise. GP had a limited role in transition: they requested the french “100% funding” for gender dysphoria and they sometimes managed hormonotherapy. Intimate subjects, such as sexuality or gender identity, were difficult to talk about with the patients, according to GP. DISCUSSION: GP seem to have a limited role in identity building of their patients. Future works should be done in order to confirm this result. Gender identity building is a complex process, which is difficult for the GP to grasp. “Peer-aid” is a concept currently being developed, that can support GP on these subjects.Les médecins généralistes (MG) sont de plus en plus confrontés aux personnes trans, car cette population augmente. L’objectif de ce travail était de comprendre les expériences de MG et de leurs patients trans face à la construction de l’identité de genre. MÉTHODE : Ce travail est une recherche qualitative sur des binômes de patients trans et leur MG. 11 entretiens semi-dirigés individuels ont été réalisés entre août 2020 et avril 2021 et analysés selon une approche phénoméno-pragmatique. RÉSULTATS : Le recours aux soins des patients interrogés est difficile, en raison d’une méfiance envers les médecins. Les patients choisissent précautionneusement leur MG et le testent. Les patients ont des attentes de médecine générale « classique », avec une place importante pour l’approche globale. La vision des MG sur la transition semble très biomédicale, avec des récits occultant la transition sociale des patients. Le questionnement identitaire est un processus continu, alors que les MG le considèrent antérieur à la transition. Le patient présente une expertise qui lui permet d’assurer la coordination du parcours de soin. Le rôle du MG dans la transition est limité à la demande ALD et parfois à la gestion de l’hormonothérapie. Les sujets intimes que sont la sexualité et l’identité de genre sont difficiles à aborder pour les MG. DISCUSSION : Les MG semblent avoir un rôle limité dans la construction identitaire de leurs patients. De futurs travaux devront être réalisés pour confirmer ce résultat. La construction de l’identité de genre est un processus complexe difficile à appréhender pour le MG. La pair-aidance est un concept qui peut accompagner les MG sur ces sujets
