État des lieux : la santé mentale des migrants, recherche des clés pour une meilleure prise en charge en médecine générale

This thesis, "Assessment: Migrants' mental health, research of the keys for a better care in general medicine" is a news topic due to the recent massive population displacements caused by the Syrian crisis. The complicated migratory journeys combined with the resistance met as they arrive in country cause in the Migrants intense psychological distress that can put some general practitioners in a difficult position. Thus, the goal of this thesis, on the basis of a litterary review, is to establish a picture of the knowledge on the Migrants' mental health so as to identify the needs in France and Lower Normandy as well as to investigate for tools and structures that could make easier the care for general practitioners. With this objective, a bibliographic research, mainly based on the databases Medline, LISSA and Santé Publique France has been realized and was analyzed the experience of a field psychiatrist through a semi-directed interview. This work highlights the reality of this mental distress with a prevalence of post-traumatic stress disorder among the Refugees that can be 10 times higher than in the occidental populations. Taking into account the cultural context seems to be an important stake that implies the implementation of trainings to ethnopsychiatry in the practitioners' path as well as a better access to profesional interpreteting. A multimodal care combined with a social integration of the Migrants is to be favored. Frequent, addictions are interesting to study for a further reasearch.Cette thèse « État des lieux : la santé mentale des migrants, recherche des clés pour une meilleure prise en charge en médecine générale », est un sujet d’actualité du fait des récents déplacements massifs de populations, engendrés notamment par la crise Syrienne. Les parcours migratoires compliqués conjugués aux résistances rencontrées à leur accueil provoque chez certains migrants une détresse psychologique intense devant laquelle des médecins généralistes peuvent parfois se retrouver en difficulté. L’objectif de cette thèse est donc, via une revue de littérature, de dresser un état de lieux des connaissances sur la santé mentale des migrants, d’identifier les besoins en France et en Basse-Normandie, ainsi que de rechercher des outils et structures pouvant faciliter la prise en charge par le médecin généraliste. À cette fin une recherche bibliographique a été réalisée principalement via les bases de données Medline, LISSA et Santé Publique France et a recueilli l’expérience d’un psychiatre de terrain via un entretien semi-dirigé. Ce travail met en évidence la réalité de cette détresse psychologique avec une prévalence de l’état de stress post traumatique chez les réfugiés pouvant être 10 fois supérieure à celle des populations occidentales. La prise en compte du contexte culturel semble être un enjeu majeur impliquant la mise en place de formations à l’ethnopsychiatrie dans le parcours des médecins ainsi que la facilitation de l’accès à l’interprétariat professionnel. Une prise en charge multimodale avec avant toute chose une intégration sociale du migrant est à privilégier. Les addictions, fréquentes, semblent intéressantes à étudier dans un futur travail
