Perception des patients ayant bénéficié d'une téléconsultation en médecine générale : étude qualitative au centre de téléconsultation de Saint-Georges-de-Rouelley (Manche, 50)

Introduction: Teleconsultation is increasing since its reimbursement has been generalized, and even more due to Covid19. Expansion of this new mean of consultation can not last in time without patients ‘acceptance. I have been focused on the teleconsultation center in Saint-Georges-de-Rouelley. Objectives: The main goal of my study was to collect patients’ feedback following a teleconsultation in General Practice. Additional goal was to highlight some ways of improvements for the dispositive and future projects of telemedicine. Method: A qualitative study by individual interviews half-directed has been carried out with 13 patients which did a teleconsultation at this center. A thematic analysis has been run independently by 2 researchers, then data processing by triangulation, until data became saturated. Results: The patients’ overall experiment is positive. They well welcomed the teleconsultation for which they see it as complementary of usual consultations. Advantages in terms of accessibility & comfort have been identified. The nurse being physically with them, connected tools quality and the fact of the project being locally implemented are working towards its acceptability. My study shows that relationship with doctor & patient is changing and that technical issues can be a source of anxiety. All patients are keen to renew the experiment. However, patients have underlined limits with teleconsultation in certain situations, mainly due to the lack of physical palpation by the doctor. Data security and loss of confidentiality are concerns expressed by the patients. Discussion: To enhance its quality & patients’ thrust towards it, the idea will be to offer training in teleconsultation to the Healthcare professionals and not to neglect material selection & internet network. Conclusion: The patients are satisfied & for the development of teleconsultation. Nevertheless, it will be useful for future projects to be very attentive regarding the protection of the relationship between the doctor and his patient which is the fundamental parameter of the medical relation. Other works could continue studying the impact of teleconsultation on the relationship between a doctor and his patient, and the triparty doctor patient nurse.Introduction : la téléconsultation est en plein essor depuis la généralisation de son remboursement et d’autant plus durant la crise de la Covid-19. Le déploiement de ce nouveau mode de consultation ne peut être durable dans le temps sans l’acceptabilité des patients. Je me suis intéressée au centre de téléconsultation de Saint-Georges-de-Rouelley. Objectifs : l’objectif principal de mon étude était de recueillir la perception des patients ayant bénéficié d’une téléconsultation au centre de Saint-Georges-de-Rouelley. L’objectif secondaire était de proposer des pistes d’amélioration pour le dispositif et les futurs projets de télémédecine. Méthode : une étude qualitative par entretiens individuels semi-dirigés a été réalisée auprès de 13 patients qui avaient eu une téléconsultation dans ce centre. Une analyse thématique a été conduite indépendamment par deux chercheurs, il s’en est suivi une triangulation des données. Une saturation des données a été obtenue. Résultats : l’expérience globale des patients est positive. Ils ont accueilli favorablement la téléconsultation qu’ils perçoivent comme complémentaire des consultations habituelles. Des avantages en termes d’accessibilité et de confort ont été identifiés. La présence de l’infirmier, la qualité des outils connectés ainsi que l’ancrage local du projet sont des atouts participant à son acceptabilité. Mon étude montre que la relation médecin-patient se modifie et que les difficultés techniques peuvent être source d’anxiété. L’ensemble des patients souhaite renouveler l’expérience. Toutefois, les patients ont identifié des limites à recourir à la téléconsultation dans certaines situations et ceci principalement à cause de l’absence de palpation par le médecin. La sécurisation des données et la perte de confidentialité sont des craintes exprimées par les patients. Discussion : pour améliorer sa qualité et la confiance que les patients lui attribuent. Il s’agira de proposer des formations en téléconsultation aux professionnels de santé et de ne pas négliger le choix du matériel et du réseau internet. Conclusion : les patients sont satisfaits et favorables au développement de la téléconsultation. Il conviendra cependant aux futurs projets de rester très attentifs à la protection du colloque singulier qui est le pilier de la relation de soin. D’autres travaux pourraient poursuivre l’étude de l’impact de la téléconsultation sur la relation médecin-patient et le trio médecin-patient-infirmier
