Intérêt et faisabilité d’une réhabilitation précoce en période post-opératoire, des compétences olfactives chez des patients opérés d’une laryngectomie totale

Total laryngectomy is a mutilating surgery whose anatomical-physiological modifications lead to an olfactory disorder, thus altering the quality of life and safety of the individual. Currently, speech therapy for these patients is mainly focused on vocal and swallowing rehabilitation despite the existence of olfactory rehabilitation methods that have proven their effectiveness. When this rehabilitation is proposed, it generally occurs only after the oncological treatments, 1 month to 20 years after the surgery. During this study, we developed a structured, progressive, and early olfactory approach, intervening at one week of post-surgical hospitalization with 5 weekly sessions. The objective is to demonstrate that this early care is feasible in postoperative conditions and that it brings a benefit for the patients. Through semi-quantitative and subjective evaluations of the operated participants, we were able to highlight significantly deficient olfactory scores in pre-operation, a drop in skills in post-operation and an improvement in olfactory capacities thanks to the rehabilitation. Patients' feelings and discomfort did not show any correlation with the olfactory scores. However, all participants were satisfied with the rehabilitation. The implementation of this care allowed us to objectify the feasibility of the treatment by the improvement of the olfactory capacities, in an early context. This study needs to be continued in the longer term and the cohort needs to be extended to compare the benefit of early treatment with late rehabilitation data from total laryngectomized patients.La laryngectomie totale est une chirurgie mutilante dont les modifications anatomo-physiologiques entraînent un trouble olfactif, altérant ainsi la qualité de vie et la sécurité de l’individu. Actuellement, la prise en soin orthophonique de ces patients est principalement axée sur la rééducation vocale et de déglutition malgré l’existence de méthodes de réhabilitation olfactive ayant prouvé leur efficacité. Lorsque cette réhabilitation est proposée, elle n’intervient généralement qu’après les traitements oncologiques soit 1 mois à 20 ans après la chirurgie. Au cours de cette étude, nous avons développé une approche olfactive structurée, progressive et précoce, intervenant à une semaine d’hospitalisation post-chirurgicale, à raison de 5 séances hebdomadaire. L’objectif est de démontrer que cette prise en soin précoce est faisable en période post-opératoire et qu’elle apporte un bénéfice pour les patients. Par l’intermédiaire des évaluations semiquantitatives et subjectives des participants opérés, nous avons pu mettre en évidence des scores olfactifs significativement déficitaires en préopératoire, une chute des compétences en post-opératoire et une amélioration des capacités olfactives grâce à la rééducation. Le ressenti et la gêne des patients n’ont pas montré de corrélation avec les scores olfactifs. Toutefois, l’ensemble des participants a été satisfait par la réhabilitation. La mise en place de ce soin nous a permis d’objectiver la faisabilité du traitement par l’amélioration des capacités olfactives, en contexte précoce.
