Rôles du pharmacien clinicien en oncologie au centre hospitalier d’Avranches-Granville

The objective of this work was to evaluate the role of the clinical pharmacist in oncology at the Avranches-Granville healthcare center. An activity report of pharmaceutical consultation at the first prescription of oral chemotherapy (OCT) was performed. 32 patients have benefited from these consultations since their implementation from December 2019 to July 2021. Drug interactions were detected in 68% of cases, particularly related to a potential decrease in the efficacy of OCT. A regional survey on OCT was conducted amongst community pharmacists, highlighting their needs, particularly in terms of formation, the town-hospital network, and the importance of information transmitted by the hospital pharmacist. The second part of this work was to evaluate the relevance of the pharmacist in oncogeriatrics and to initiate his participation in multidisciplinary oncogeriatric assessments. Only 4 patients benefited from an oncogeriatric consultation during the study period. Nevertheless, 2.75 pharmaceutical interventions could be performed per patient. A retrospective study of 17 files highlighted the interest of integrating the hospital pharmacist into these consultations, particularly in terms of reviewing the relevance of drug treatments.L’objectif de ce travail a été d’évaluer le rôle du pharmacien clinicien en oncologie au centre hospitalier d’Avranches-Granville. Un bilan de l’activité de consultation pharmaceutique de primo-prescription de chimiothérapie orale (CTO) a été réalisé. 32 patients ont bénéficié de ces consultations depuis leur instauration de décembre 2019 à juillet 2021. Des interactions médicamenteuses ont été détectées dans 68% des cas, notamment liées à une baisse potentielle de l’efficacité de la CTO. Une enquête régionale sur les CTO réalisée auprès des pharmaciens d’officine a permis de mettre en évidence leurs besoins, surtout en termes de formation, de lien ville-hôpital, et l’importance des informations transmises par le pharmacien hospitalier. La seconde partie de ce travail a été d’évaluer la pertinence du pharmacien en oncogériatrie et d’initier sa participation aux évaluations oncogériatriques pluridisciplinaires. Seulement 4 patients ont pu bénéficier d’une consultation d’oncogériatrie sur la période de l’étude. Néanmoins, 2,75 interventions pharmaceutiques ont pu être proposées par patient. Une étude rétrospective sur 17 dossiers a mis en évidence l’intérêt d’intégrer le pharmacien hospitalier à ces consultations notamment en termes de revue de pertinence des traitements médicamenteux
