DISCREPANCIES IN THE USE OF PHARMACOLOGICAL STENTS IN DIABETIC PATIENTS WITH STEMI ASSISTED BY SUS AND BY THE PRIVATE SECTOR – VICTIM REGISTRY

Percutaneous coronary angioplasty (PCI) with stent placement is one of the main options in the treatment of ST-segment elevation acute myocardial infarction (STEMI). Diabetics, however, represent a special patient population due to the greater difficulty of percutaneous treatment, with high rates of restenosis and unfavorable clinical outcomes when using conventional stents compared to pharmacological stents. Thus, although they have a high cost, the SUS system has been allowed to use pharmacological stents in these special patients since 2014. Objective: Evaluate the use of pharmacological stents in patients with STEMI in the general population and in diabetic patients assisted by SUS and by the private sector in Sergipe; monitor if the recommendations in the national legislation for the adequate use of this device are being followed in our State. Methodology: This is a cross-sectional study with a quantitative approach using the VICTIM registry data. Data were collected in the four hospitals with the capability to perform primary PCI in Sergipe, from December 2014 to March 2017. Results: A total of 707 patients diagnosed with STEMI were evaluated, of which 589 were assisted by the public service and 118 by the private sector. The use of pharmacological stents in the general population was lower in the public service compared to the private sector, during both primary and non-primary angioplasties (10.5% vs 82.4% in primary PCI, 9.3% vs 85.7% in non-primary PCI, p <0.001). In diabetic patients, results were similar, with predominance of use in the private sector (8.7% vs 90.6% in primary PCI, 11.9% vs 100% in non-primary PCI). Conclusion: This study reveals a great disparity in the use of pharmacological stents during IPC between SUS and the private sector, in both the general population or in diabetic patients. Thus, even after the use in the SUS system was allowed, recommendations for the use of pharmacological stents have not been followed in Sergipe. These findings demonstrate the challenges that need to be overcome in order to achieve improvements in the quality of services provided by the public health system.A angioplastia coronariana percutânea (ICP) com colocação de stents representa uma das principais opções no tratamento do infarto agudo do miocárdio com supradesnivelamento do segmento ST (IAMCSST). Os diabéticos, entretanto, representam população especial de pacientes pela maior dificuldade de tratamento percutâneo, com altas taxas de reestenose e desfechos clínicos desfavoráveis, quando utilizam stents convencionais (SC) comparados aos farmacológicos. Dessa forma, embora apresentem elevado custo, os stents farmacológicos (SF) foram liberados para uso no SUS nessa parcela especial de doentes desde o ano de 2014. Objetivos: Avaliar a taxa de uso de stents farmacológicos em pacientes com IAMCSST na população geral e em diabéticos assistidos pelo SUS e pela rede privada em Sergipe, além de analisar se as recomendações da legislação nacional para uso adequado desse dispositivo foram implementadas em nosso Estado. Metodologia: Trata-se de um estudo transversal com abordagem quantitativa que utilizou os dados do Registro VICTIM. Os dados foram coletados nos quatro únicos hospitais com capacidade para realizar ICP primária em Sergipe, no período de dezembro de 2014 a março de 2017. Resultados: Foram avaliados 707 pacientes diagnosticados com IAMCSST, dos quais 589 foram atendidos pelo serviço público, e 118 pelo privado. O uso de SF na população geral foi menor no serviço público comparado ao privado, tanto durante as ICP primárias, quanto em não primárias (10,5% no vs 82,4% na ICP primária, 9,3% vs 85,7% na ICP não primária, p< 0,001). Nos diabéticos, os resultados são semelhantes, com predomínio de utilização ainda no serviço privado (8,7% vs 90,6% na ICP primária, 11,9% vs 100% na ICP não primária). Conclusão: O estudo revela grande disparidade no uso de SF durante a realização da ICP entre SUS e privado, seja na população geral ou nos diabéticos. Assim, mesmo após a liberação para uso no SUS, as recomendações para utilização de SF não têm sido seguidas em Sergipe. Tais achados demonstram os desafios que necessitam ser vencidos para que possamos atingir melhorias na qualidade dos serviços prestados em saúde pública.Aracaju, S
