Aerobic exercise or noninvasive ventilation, which one is more effective against the decline in lung function in cystic fibrosis patients? A systematic review

BACKGROUND: Cystic Fibrosis (CF) or mucoviscidosis is a genetic disease caused by changes in the long arm of chromosome 7 that causes defects in the production of CFTR (Cystic Fibrosis Transmembrane Transmittance Conductance). pulmonary. Physical therapy makes it necessary in this scenario to use therapies such as aerobic exercise and NIV. OBJECTIVE: To conduct a systematic review to determine which therapeutic therapy between NIV and aerobic exercise is most effective in reducing the deterioration of lung function in patients with cystic fibrosis. DATA SOURCES: A literature search using the following databases: Scopus, MEDLINE-PubMed, Web of Science, PEDRO, and Science Direct. STUDY SELECTION: Clinical trials of a cystic fibrosis patient who used NIV or performed aerobic exercise to promote positive changes in lung function. SELECTION CRITERIA: Randomized controlled trials of patients with chemical fibrosis who use NIV or perform aerobic exercise to promote positive changes in lung function. RESULTS: A total of 2,701 studies were used, with only 8 included in the inclusion of the present study, with 1 added in the search manual, ending with 9 studies. Regarding the therapies used, there was a numerical superiority in relation to the studies that used aerobic exercises (6 studies), while those that used the NIV (3 studies) were smaller. CONCLUSION: Aerobic exercise protocols demonstrated better results in pulmonary function in CF patients than studies using NIV as a treatment.INTRODUÇÃO: A Fibrose Cística (FC) ou mucoviscidose é uma doença genética causada por alteração no braço longo do cromossomo 7 que leva a defeitos na produção da Cystic Fibrosis Transmembrane Conductance Regulator (CFTR), estas alterações geram disfunções em vários sistemas, dentre eles o pulmonar. A fisioterapia faz-se necessária neste cenário, podendo utilizar de terapias como o exercício físico aeróbico e a VNI. OBJETIVOS: Conduzir uma revisão sistemática para verificar qual modalidade terapêutica dentre a VNI e o exercício físico aeróbico é mais eficaz na redução da deterioração da função pulmonar de pacientes com fibrose cística. ESTRATÉGIA DE BUSCA: Uma busca na literatura utilizando as seguintes bases de dados: Scopus, MEDLINE-PubMed, Web of Science, PEDRO e Science Direct. CRITÉRIO DE SELEÇÃO: Ensaios clínicos randomizados de paciente com fibrose cística, que utilizavam VNI ou realizavam exercícios aeróbicos a fim de promover alterações positivas na função pulmonar. RESULTADOS: Um total de 2.701 estudos foram inicialmente identificados, com apenas 8 que estavam nos critérios de inclusão do presente estudo, sendo adicionado 1 na busca manual, finalizando com 9 estudos. Referente às terapias utilizadas houve uma superioridade numérica quanto aos estudos que utilizaram exercício aeróbico (6 estudos), enquanto aqueles que utilizaram VNI (3 estudos) foram minoria. CONCLUSÃO: Os protocolos de exercício aeróbico demonstraram melhores resultados na função pulmonar dos pacientes com FC do que aqueles estudos que utilizaram a VNI como tratamento.Lagart
