A 56-year-old man presented with diffuse preauricular swelling of 20 months’ duration, which was caused by a symptomatic ectopic third molar in the right subcondylar region. There was no lymphadenopathy or paresis of the facial nerve. An orthopantogram (OPG) showed an ectopic third molar with associated unilocular radiolucency (Fig. 1) and computed tomography (CT) showed a 26 mm × 11 mm lesion perforating both, lateral and medial ramus cortices (Fig. 2)
