Influence of maternal body mass index and excessive gestational weight gain on maternal and fetal risk factors and their influence on newborns early motoric development

Uvod: U potencijalne faktor rizika, pored indeksa telesne uhranjenosti majke pre ostvarivanja trudnoće ubraja se i prekomerni prirast u telesnoj težinu u trudnoći. U zavisnosti od vrednosti ova dva pokazatelja, definisani su i proporcionalni rizici unutra svake grupe uhranjenosti. Problematika gojaznosti nosi izazove u svakodnevnom radu akušera, obzirom da se dovodi u vezu sa produženim vremenom trajanja porođaja, učestalijom indukcijom i stimulaciom porođaja, prekomernim gubitkom krvi u porođaju i učestalijim operativnim završetkom porođaja kod neuspelih indukcija i prolongiranih porođaja. Pored komplikacija u trudnoći i porođaju sa kojima se dovodi u vezi, gojaznost majke može dovesti i do odstupanja i u finom motoronom razvoju deteta i prepoznata je kao rizikofaktor za odstupanja u neurokognitivnom i razvoju finih motornih veština. Ciljevi: Istraživanjem je definisano pet ciljeva. Prvi cilj je da se opišu maternalni komorbiditeti, morbiditeti nastali u toku trudnoće, antropometrijski i laboratorijski parametri u trudnoći, kao i fetalni ultrasonografski i antropometrijski parametri novorođenčeta i motorna zrelost deteta. Drugi cilj je da se utvrdi povezanost prekoncepcijske gojaznosti i prekomernog prirasta telesne težine u trudnoći na faktore rizika u trudnoći. Treći cilj je da se ispita postojanje uticaja sinergističkog delovanja prekoncepcijske gojaznosti i prekomernog prirasta telesne težine u trudnoći na pojavu i stepen ispoljenosti faktora rizika u trudnoći. Četvrti cilj je da se utvrde potencijalni prediktori ispitivanih faktora rizika iz skupa podataka maternalnih, fetalnih i neonatalnih parametara kod majki sa prekoncepcijskom gojaznošću i prekomernim prirastom u telesnoj težini u trudnoći. Peti cilj je da se utvrdi potencijalna povezanost prekoncepcijske gojaznosti i prekomernog prirasta telesne težine u trudnoći na rani motorni razvoj deteta. Metod: Studija je koncipirana po tipu prospektivne, opservacione, kliničke studije i u nju je uključeno 200 ispitanica podeljenih u dve jednake grupe, studijsku i kontrolnu. Ispitanice u studijskoj grupi pre ostvarivanja koncepcije imale su povišene vrednosti indeksa telesne uhranjenosti ili su imale prekomerni prirast u telesnoj težini u trudnoći ili oba, dok su ispitanice u kontrolnoj grupi pre ostvarivanja trudnoće imale vrednosti indeksa telesne uhranjenosti u granicama referentnih vrednosti i nisu imale prekomerni prirast u telesnoj težini u trudnoći...Introduction: Increased pre- pregnancy body mass index as well as excessive gestational weight gain is potential risk factor for pregnancy course. Depending on the value of these two indicators, proportional risks within each nutrition group are defined. The complexity of obesity brings challenges to the obstetrician’s daily practice, since it is associated with prolonged labor, more frequent induction and stimulation of labor, excessive blood loss during delivery and more frequent cesarean sections in cases of failed inductions and prolonged labors. In addition to the complications related to pregnancy and delivery, maternal obesity can lead to abnormalities in the early motoric development of the newborns and it is recognized as a risk factor for abnormalities in neurocognitive development and early motoric development. Objectives: Five objectives are defined in this research. The first objective is to describe maternal comorbidities, morbidities occurring during pregnancy, anthropometric and laboratory parameters in pregnancy, as well as fetal ultrasonographic and anthropometric parameters of the newborn and motoric development of the child. The second objective is to determine the connection between pre- pregnancy obesity and excessive gestational weight gain to risk factors in pregnancy. The third objective is to examine the influence of the synergistic effect of pre- pregnancy obesity and excessive gestational weight gain on the occurrence and degree of manifestation of risk factors in pregnancy. The fourth objective is to determine the potential predictors of the observed risk factors from the data set of maternal, fetal and neonatal parameters in mothers with pre- pregnancy obesity and excessive gestational weight gain. The fifth objective is to determine the potential connection between pre- pregnancy obesity and excessive gestational weight gain to the early motoric development of the newborn. Method: The study was designed as prospective, observational, clinical study and it included 200 participants divided into two equal groups, study and control group. Participants in the study group had elevated values of pre- pregnancy body mass index or had excessive gestational weight gain or both, while participants in the control group had values of body mass index within the reference values before conception and did not have excessive gestational weight gain..
