Prediction of response to Cardiac Resynchronization Therapy through the analysis of baseline electrocardiographic variables

INTRODUÇÃO: A terapia de Ressincronização Cardíaca (TRC) apresenta benefícios bem estabelecidos em paciente que apresentam Insuficiência cardíaca (IC) com fração de ejeção reduzida e QRS largo. Apesar dos resultados positivos, aproximadamente 30% dos pacientes não obtém resposta clínica satisfatória com uso da TRC mesmo seguindo rigorosos protocolos de indicação. OBJETIVOS: Identificar variáveis eletrocardiográficas e vetorcardiográficas previamente ao implante dispositivos de ressincronização cardíaca com potencial de predição da resposta à TRC. MÉTODOS: A população estudada foi composta por pacientes com indicação de TRC encaminhados ao Serviço de Estimulação Cardíaca Artificial do Instituto Dante Pazzanese de Cardiologia. Foram incluídos pacientes com IC em terapia clínica otimizada, classes funcionais II, III e IV da NYHA, fração de ejeção do ventrículo esquerdo 35% e com QRS apresentando duração maior que 150 ms e morfologia sugestiva de bloqueio de ramo esquerdo. Após seis meses do implante do ressincronizador, os pacientes foram classificados por critérios clínicos e ecocardiográficos em respondedores e não-respondedores à TRC. Variáveis extraídas do ECG basal foram estudadas em relação às diferenças entre os grupos de pacientes. RESULTADOS: Foram incluídos e completaram o seguimento previsto trinta e dois pacientes. As áreas sobre a curva das derivações V5, V6 e X apresentaram-se significativamente menores no grupo de pacientes não-respondedores. O Índice S, proposto no presente estudo, demonstrou-se significativamente maior entre os pacientes respondedores à TRC. O Critério Combinado de Dissincronia apresentou alta sensibilidade e especificidade em relação a predição de resposta à TRC. CONCLUSÃO: Os resultados desta pesquisa apontam para existência de diferenças no eletrocardiograma basal entre os pacientes que posteriormente obtiveram boa resposta ou resposta inadequada à TRC. O critério descrito no presente trabalho elevou de forma importante a especificidade para predição à TRC sem reduzir a sensibilidade, mostrando alto valor preditivo negativo e boa acurácia.BACKGROUND: Cardiac resynchronization therapy (CRT) is an effective and well-established treatment for patients with Heart Failure (HF), reduced Ejection Fraction and wide QRS. Despite the positive results, approximately 30% of the patients did not obtain a satisfactory clinical response with CRT even with rigorous indication protocols. OBJECTIVES: To identify electrocardiographic and vectorcardiographic variables prior to resynchronization devices implantation with potential to predict the response to CRT. METHODS: The study population consisted of patients with indication of CRT referred to cardiac stimulation departament of the Dante Pazzanese Heart Institute. There were included HF patientes in optimized clinical therapy, NYHA functional classes II, III and IV, left ventricular ejection fraction 35% and QRS 150 ms with morphology of left bundle branch block. After six months of follow-up, patients were classified by clinical and echocardiographic criteria in responders and non-responders to CRT. Variables extracted from baseline ECG were studied for differences in the groups. RESULTS: Thirty-two patients were included and completed the follow-up. The areas under the curve of the V5, V6 and X leads were significantly lower in the non-responders group. The S Index, proposed in the present study, was shown to be significantly greater among patients responding to CRT. The Combined Dyssynchrony Criteria presented high sensitivity and specificity in relation to the prediction of response to CRT. CONCLUSION: The results of this study show differences in the baseline electrocardiogram of patients who obtained a good response or an inadequate response to CRT. The criterion described in the present study significantly increased specificity for predicting CRT without reducing sensitivity, showing high negative predictive value and good accuracy
