Most common functional gastrointestinal disorders in infants and their relationship with nutrition Part I. Colic and regurgitation

Functional gastrointestinal disorders in infants and young children are one of the most common gastroenterological problems in this age group. Characteristic feature of this disorders is the lack of organic cause in the organs. The paper presents the definition of regurgitation and intestinal colic (according to the III Rome criteria), discusses the etiopathology of these problems and summarises the contemporary therapeutic methods. In addition to behavioural and pharmaceuticals (in lesser extent) methods, nutrition habits play a very important role in the treatment method. Particularly emphasized is the role of diet, both child and mother. Properly chosen diet not only ensures proper development of the child, but it is also an important element in the treatment of the above functional disorders. Among the dietary treatments for infantile colic the best documented evidence of the effectiveness was proved to extensively hydrolysed formulas for artificially- fed babies, and the use of maternal hypoallergenic diet for breast-fed babies. The best results were achieved with children which had documented food hypersensitivity. Soy proteins and lactose-reduced formulas became to be ineffective in relieving colic. So commonly used herbal teas moderately mitigated symptoms but because of the fear of losing thirst they are not recommended. Nutrition habits can be used as one of the methods to alleviate regurgitation problems. Most of all it is important to correctly place the baby during and after feeding. The extensively hydrolysed formulas and, in selected cases, thickening formulas that decrease volume of lost food, are beneficial in regurgitations in infants. Special attention should be paid to rate of weight gain and overall infant development in case of both: infantile colic symptoms and regurgitation. In the case of disturbance in the harmonious development of the infant, one should be looking for some other causes of disorder, especially outbreaks of infection in the urinary tract, gastrointestinal tract or states that require surgical interventions (including pylorostenosis or intussusception).Zaburzenia czynnościowe przewodu pokarmowego u niemowląt i młodszych dzieci są jednym z najczęstszych problemów z zakresu gastroenterologii w tej grupie wiekowej. Charakterystyczną cechą tych zaburzeń jest brak podłoża organicznego. W pracy przedstawiono definicję kolki jelitowej i ulewania zgodnie z III kryteriami rzymskimi, omówiono przyczyny tych dolegliwości oraz zestawiono próby dotychczas stosowanych działań terapeutycznych. Obok metod behawioralnych oraz, w mniejszym stopniu, farmakologicznych bardzo ważną rolę w leczeniu odgrywają metody żywieniowe. Szczególnie podkreślono rolę stosowania odpowiedniej diety zarówno u dziecka, jak i u matki. Prawidłowo dobrana dieta nie tylko zapewnia prawidłowy rozwój dziecka, ale także jest istotnym elementem w terapii powyższych zaburzeń czynnościowych. Wśród żywieniowych metod leczenia kolki niemowlęcej najlepiej udokumentowaną skuteczność mają hydrolizaty o znacznym stopniu hydrolizy w przypadku dzieci karmionych sztucznie oraz stosowanie u matki diety hipoalergicznej w przypadku dzieci karmionych piersią. Przynosiło to najlepsze efekty u dzieci z udokumentowaną nadwrażliwością pokarmową. Nieefektywne w łagodzeniu kolki okazały się preparaty sojowe oraz preparaty o zmniejszonej zawartości laktozy. Często stosowane pojenie herbatami ziołowymi w umiarkowanym stopniu łagodzi objawy, ale z obawy przed utratą pragnienia wskutek ich podawania nie są to napoje zalecane. Metody żywieniowe znajdują również zastosowanie w łagodzeniu problemów regurgitacji. Istotne znaczenie ma tu przede wszystkim prawidłowe ułożenie dziecka podczas karmienia i po nim. Korzystne okazały się preparaty o znacznym stopniu hydrolizy, a w uzasadnionych przypadkach – mieszanki mleczne z dodatkiem środków zagęszczających, które zmniejszają objętość traconego w ten sposób pokarmu. Zarówno w przypadku występowania objawów kolki niemowlęcej, jak i ulewań należy zwrócić szczególną uwagę na tempo przyrostu masy ciała i ogólny rozwój dziecka. W przypadku zaburzeń w harmonijnym rozwoju niemowlęcia należy poszukiwać innej przyczyny dolegliwości, przede wszystkim ognisk zakażenia w układzie moczowym, pokarmowym czy stanów wymagających interwencji chirurgicznych, między innymi pylorostenozy czy wgłobienia jelit
