Oral health and its relationship with the self-perception of the state of health in older adults : secondary analysis of the sabe-Colombia survey

La salud oral hace parte fundamental de la salud general y es claro que tiene un gran impacto, tanto en salud general como en aspectos relacionados con nutrición, salud metal, emociones y ámbito social. Objetivo: Con este trabajo, a partir del análisis de datos de la encuesta SABE Colombia realizada en el año 2015 se pretende conocer cuál es la asociación entre la salud oral y la autopercepción de salud, en la población colombiana. Diseño: Es un trabajo de corte transversal, a partir de los datos de la encuesta SABE Colombia, que incluyó 23.694 sujetos de 60 años o más teniendo representatividad de todo el país. En nuestro trabajo analizamos un total de 18.180 sujetos que contaban con la valoración completa de salud oral y de autopercepción de la salud general. Medidas: Se evaluó el estado de salud autopercibido mediante auto informe, el estado de salud oral se evaluó a través del índice GOHAI (Geriatric Oral Health Assesment Index) y por preguntas sobre la cantidad de dientes naturales y el uso de dispositivos protésicos fijos o removibles. Resultados: De los 18.180 sujetos incluidos en el análisis final 1,939 (10,6%) informaron un estado de salud autopercibido deficiente. En el análisis multivariado, encontramos una asociación entre una puntuación baja de GOHAI y la ausencia total de dientes (OR 1.21, IC 95% 1.06 – 1.38). A demás el uso de prótesis, especialmente fijas, favoreció una mejor autopercepción del estado de salud. Conclusiones: En la población colombiana mayor, los factores dados por el edentulismo total y los puntajes bajos en el GOHAI fueron los más relevantes asociados con la deficiente salud autopercibida. El uso de prótesis dentales se asoció con una mejor autopercepción de salud. Esta investigación muestra el importante impacto de la salud bucal en el estado general de salud en personas mayores.Oral health is a fundamental part of general health and it has a great impact, both in general health and in aspects related to nutrition, metal health, emotions, and social areas. Measures: Self-perceived health status was assessed by self-report, oral health status was assessed through the GOHAI index (Geriatric Oral Health Assessment Index) and by questions about the number of natural teeth and the use of fixed prosthetic devices or removable. Results: Of the 18,180 subjects included in the final analysis, 1,939 (10.6%) reported on poor self-perceived health status. In the multivariate analysis, we found an association between a low GOHAI score and the total absence of teeth (OR 1.21, 95% CI 1.06 - 1.38). In addition, the use of prostheses, especially fixed ones, favored a better self-perception of the state of health. Conclusions: In the older Colombian population, the factors given by total edentulism and low GOHAI scores were the most relevant associated with poor self-perceived health. The use of dental prostheses is associated with a better self-perception of health.
