Pour une amélioration de la prise en charge du tabagisme des patients psychiatriques : identification des forces, faiblesses, opportunités et menaces, et propositions

Patients with mental illness are most often “Hard Core Smokers”. Smoking is their main cause of death and reduces their life expectancy by 25 years. The decline in smoking observed in the general population between 2014 and 2019 was significantly less for these patients. Since 2020, the prevalence of smoking has stabilized in the context of the current health, psychological, economic and social crisis, to which they are more vulnerable. In view of this, the objective of our study was to identify the strengths, weaknesses, opportunities and threats in helping people quit smoking based on their perception in order to offer them the most appropriate support methods. We first have done inventory of the literature, which we completed through a qualitative study with patients hospitalized in psychiatry at the CHAI (Centre Hospitalier Alpes Isère). We conducted 14 semi-structured interviews which were transcribed and then analyzed using the method of interpretative phenomenological analysis (IPA). The main strengths of patients are their desire to quit, their awareness of the issues, their good knowledge of resources and their interest in reduction. Weaknesses to work on are their strong belief that quitting is just a matter of willingness, their negative self-perception, and their rejection of aids. According to the patients, hospitalization can represent an opportunity or a threat. It is most often experienced with difficulty and leads to an increase in smoking. But for some, it can represent an opportunity to stop or reduce: through access to addiction care for patients with another addiction, or distance from difficult living conditions. The hospital organization can also represent an opportunity, especially by the protective character of the restrictions. However, it contributes to the negative experience of hospitalization for the most restricted patients, and reinforces the negative self-perception of the most isolated, precarious, or vulnerable patients. In order to improve care, in addition to what is usually proposed, we recommend addressing the possibility of a reduction and accompanying the prescription of Nicotinic Replacement Therapy (NRT) with precise explanations on their functioning. Close follow-up and psychotherapy must be combined. At the hospital, there must be additional care as soon as they arrive to limit the increase in consumption and best support patients for whom hospitalization is an opportunity, with regular reassessment. Based on those of patients, we make proposals for improvement in the hospital along two lines: firstly, to make tobacco management a priority (by means of explanatory posters, discussion groups, etc.), secondly, to motivate patients with incentive proposals (access to a workshop if stopping procedure, etc.) Our work makes it possible to understand the resistance of certain patients about the tobacco-free psychiatric hospital. However, it brings a strong argument in its favor: the smoking ban would improve the experience of hospitalization for patients in difficulty with the current organization.Les patients atteints de pathologies psychiatriques sont le plus souvent des « Hard Core Smokers ». Le tabagisme représente leur principale cause de mortalité avec une réduction de leur espérance de vie pouvant aller jusqu’à 25 ans. La baisse du tabagisme observée dans la population générale entre 2014 et 2019 était significativement moindre chez ces patients. Depuis 2020, la prévalence du tabagisme s’est stabilisée dans le contexte de l’actuelle crise sanitaire, psychologique, économique et sociale, face à laquelle ils sont plus vulnérables. Devant ce constat, l’objectif de notre étude était d’identifier les forces, les faiblesses, les opportunités et les menaces dans l’aide à l’arrêt du tabac à partir de leur perception pour leur proposer des modalités d’accompagnement les plus adaptées. Nous avons d’abord procédé à un état des lieux de la littérature, que nous avons complété par une étude qualitative réalisée auprès de patients hospitalisés en psychiatrie au CHAI (Centre Hospitalier Alpes Isère). Nous avons réalisé 14 entretiens semi-structurés qui ont été retranscrits puis analysés selon la méthode de l’analyse interprétative phénoménologique. Les principales forces des patients sont leur désir d’arrêter, leur conscience des enjeux, leur bonne connaissance des ressources et leur intérêt pour la diminution. Les faiblesses, sur lesquelles travailler, sont leur forte croyance que l’arrêt n’est qu’une question de volonté, leur perception négative de soi et leur rejet des aides. Selon les patients, l’hospitalisation peut représenter une opportunité ou une menace. Elle est le plus souvent vécue difficilement et entraîne une augmentation du tabagisme. Mais pour certains, elle peut représenter une opportunité pour un arrêt ou une diminution : par l’accès aux soins addictologiques pour les patients ayant une autre addiction, ou la mise à distance avec des conditions de vie difficiles. L’organisation hospitalière peut également représenter une opportunité, surtout par le caractère protecteur des restrictions. Cependant, elle contribue au vécu négatif de l’hospitalisation pour les patients subissant le plus les restrictions, et renforce la perception négative de soi des patients les plus isolés, précaires, ou vulnérables. Afin d’améliorer leur prise en charge, en plus de ce qui est habituellement proposé, nous préconisons d’aborder la possibilité d’une diminution et d’accompagner la prescription des Traitements Nicotiniques de Substitution (TNS) d’explications précises sur leur fonctionnement. Un suivi rapproché ainsi qu’une psychothérapie doivent être associés. A l’hôpital, doit s’ajouter une prise en charge dès leur arrivée pour limiter l’augmentation des consommations et accompagner au mieux les patients pour qui l’hospitalisation est une opportunité, avec réévaluation régulière. En nous basant sur celles des patients, nous faisons des propositions d’amélioration à l’hôpital selon deux axes : premièrement faire de la prise en charge du tabac une priorité (par le moyen d’affiches explicatives, de groupes de paroles…), deuxièmement motiver les patients par des propositions incentives (accès à un atelier si démarche d’arrêt…). Notre travail permet de comprendre les résistances de certains patients à propos de l’hôpital psychiatrique sans tabac. Cependant il apporte un argument fort en sa faveur : l’interdiction de fumer améliorerait le vécu de l’hospitalisation des patients en difficulté avec l’organisation actuelle
