Objetivos de presión arterial en el periodo perioperatorio en pacientes sometidos a cirugía no cardíaca

The management of blood pressure in the perioperative period has acquired new relevance in recent times. This article reviews the BP targets in the preoperative, intraoperative, and postoperative periods. In the preoperative period, recent publications minimize the relevance of hypertension to give great relevance to hypotension as a predictor of negative outcomes. Studies evaluating the intraoperative period conclude that absolute MAP thresholds close to 65 mmHg, together with the time below these thresholds, are useful for predicting mortality,myocardial damage after non-cardiac surgery, acute kidney injury, and possibly delirium and cognitive impairment. On the other hand, in the postoperative period there are more shadows than lights since the limitations of the scenario, that is, the intermittence of BP control, together with the lack of studies in this specific make clinical decision-making difficultEl manejo de la presión arterial en el periodo perioperatorio ha cobrado una nueva relevancia en el último tiempo. El presente artículo tiene por objetivo realizar una revisión de los objetivos de PA en los períodos preoperatorio, intraoperatorio y postoperatorio. En el periodo preoperatorio las publicaciones recientes minimizan la relevancia de la hipertensión arterial para darle una gran relevancia a la hipotensión arterial como predictor de desenlaces negativos. Los estudios que evalúan el período intraoperatorio concluyen que umbrales absolutos de PAM cercanos a los 65 mmHg, junto con el tiempo bajo dichos umbrales, son útiles para predecir mortalidad, daño miocárdico luego de cirugía no cardíaca, daño renal agudo y posiblemente delirium y deterioro cognitivo postoperatorio. Por otro lado, en el período postoperatorio hay más sombras que luces ya que las limitaciones del escenario, vale decir, la intermitencia del control de la PA, junto con la falta de estudios en este período en específico dificultan la toma de decisiones clínica
