Políticas efectivas para reducir la mortalidad infantil en el Perú: ¿Cómo reducir la mortalidad infantil en las zonas más pobres del país?

The purpose of this paper is to find the most effective determinants in reducing child and neonatal mortality, so that policymakers may use policy variables to achieve Millennium Development Goal (MDG) 4: the reduction of child mortality rates by two-thirds by the year 2015 from a baseline of 1992. The paper uses two surveys: the IV Demographic and Family Health Survey (DFHS, 2000), as well as the II Census of Sanitary Infrastructure and Resources of the Health Sector for the year 1999. The duration models (Cox Proportional Hazard model) provided the main methodology in finding the determinants ofneonatal and child mortality, since this is directed towards the survival trajectory of a child in the infant period; in other words, the longer a child manages to stay alive, the better his chances of survival. The evidence suggests that the determinants affecting the neonatal stage most are the healthcare practices for both mother and child: the exclusive use of breastfeeding during the first month after birth, giving birth at a health center, and the provision of health insurance for both. With regard to infant mortality, the results underline the importance of giving birth at a health center, following the vaccination schedule for children of this age, following the number of growth controls, as well as the months of exclusive breastfeeding infants receive.El propósito del presente trabajo es hallar aquellos determinantes más efectivos para reducir la mortalidad neonatal e infantil, para que sean utilizados como variables de política por el «policymaker» y se pueda alcanzar el Objetivo de Desarrollo del Milenio # 4: reducir para el año 2015 la tasa de mortalidad infantil a un tercio de su valor inicial de 1992. El trabajo hace uso de dos bases de datos: la IV Encuesta Demográfica y de Salud Familiar (Endes, 2000) y el II Censo de Infraestructura Sanitaria y Recursos del Sector Salud de 1999. La metodología principal que se utiliza para hallar los determinantes de la mortalidad neonatal e infantil son los modelos de duración (riesgos proporcionales de Cox), por ser los que más se ajustan con la trayectoria de supervivencia de un niño en el período infantil; es decir, cuanto más tiempo un niño logre permanecer vivo, sus probabilidades de morir disminuyen. Existe evidencia suficiente para afirmar que aquellos determinantes que más afectan a la etapa neonatal son las prácticas de cuidado del binomio madre – hijo; es decir, los factores más importantes son la lactancia exclusiva durante el primer mes, el parto institucional y el seguro de salud para ambos. En relación con la mortalidad infantil, los resultados refuerzan la importancia del parto institucional, del seguimiento del esquema de vacunación de los niños a esta edad, del número de controles de crecimiento, así como del número de meses de lactancia exclusiva que reciben
