Towards improved opioid analgesia and central nervous system drug delivery : Focus on glymphatic system, morphine-3-glucuronide and peripheral opioid receptors

Opioids are effective drugs in the treatment of moderate-to-severe pain but development of tolerance, addiction, and severe adverse effects limit their use. The effects of opioids are mediated through opioid receptors that are widely expressed in the central nervous system (CNS) and in the periphery. However, only certain anatomical locations are essential for antinociception and analgesia. Adverse effects may be reduced by targeting opioid exposure to locations important for opioid antinociception and analgesia. The aim of this thesis was to investigate new possibilities to improve opioid analgesia by clarifying the role of pronociceptive morphine-3-glucuronide (M3G) in morphine tolerance, by assessing the role of peripheral opioid receptors in antinociception and development of tolerance, and by enhancing opioid delivery to the CNS by utilizing the glymphatic system with dexmedetomidine or systemic hypertonic saline. Nociceptive effects were determined with common mechanical and thermal pain models in Sprague-Dawley rats. Morphine, M3G, oxycodone, naloxone, methylnaltrexone, naltrexone, and dexmedetomidine were administered through systemic, lumbar intrathecal, and intracisternal routes. Proinflammatory glial activation and levels of excitatory neurotransmitter Substance P in spinal cord were determined with immunohistochemistry. Drug concentrations were quantified from blood, cerebrospinal fluid, CNS tissue, and microdialysis samples, using liquid chromatography-tandem mass spectrometry. Intrathecal M3G caused tactile allodynia and induced the development of cross-tolerance to morphine. Both morphine and M3G increased Substance P levels but had no effect on activation of microglia or astrocytes in the dorsal horn of the spinal cord. While the peripherally restricted opioid receptor antagonist methylnaltrexone had no effect on morphine antinociception or development of tolerance, the CNS permeable opioid receptor antagonist naltrexone prevented both phenomena. Co-administration of α2-adrenergic agonist dexmedetomidine increased brain and spinal cord concentrations of intracisternally administered oxycodone and naloxone. Also, co- administration of systemic hypertonic saline enhanced the spinal delivery of lumbar intrathecal morphine and increased its antinociceptive efficacy. These results demonstrate that M3G independently promotes pronociceptive effects and that 5-days of repeated exposure induces cross-tolerance to morphine. Antagonism of peripheral opioid receptors affected neither acute morphine antinociception nor the development of tolerance in mechanical and thermal pain models. Glymphatic-enhanced drug delivery by dexmedetomidine or hypertonic saline is a potential strategy to increase brain and spinal cord concentrations and to improve efficacy of intrathecal opioids.Opioidit ovat tehokkaita kipulääkkeitä, mutta haittavaikutukset ja toleranssin kehittyminen rajoittavat niiden käyttöä. Opioidien vaikutukset välittyvät opioidireseptorien kautta keskushermostossa ja kehon perifeerisissä osissa. Kohdistamalla opioidi kivunhoidollisesti tärkeille anatomisille alueille, voidaan mahdollisesti parantaa opioidien hyöty-haittasuhdetta ja vähentää haittavaikutuksia. Myös opioidien aktiiviset aineenvaihduntatuotteet, kuten morfiinin päämetaboliitti morfiini-3-glukuronidi (M3G) voivat heikentää kivun hoidollista vastetta. Lisäksi glymfaattinen järjestelmä, joka lisää aivo-selkäydinnesteen virtausta keskushermoston syvempiin rakenteisiin, voidaan mahdollisesti hyödyntää lääkeaineiden saatossa keskushermostoon. Tämän prekliinisen tutkimuksen tavoitteena oli selvittää rotilla suoritetuilla käyttäytymiskokeilla ja pitoisuusmäärityksillä toistuvan M3G-altistuksen vaikutusta kivulle herkistymiseen, sekä veri-aivoesteen ulkopuolella sijaitsevien opioidireseptorien merkitystä kivun hoitoon ja morfiinitoleranssin kehittymiseen. Lisäksi tutkimuksessa selvitettiin α2-agonisti deksmedetomidiinilla tai systeemisellä hypertonisella suolaliuoksella (HTS) aktivoidun glymfaattisen sisäänvirtauksen hyödyntämistä opioiden saatossa keskushermostoon. Lukinkalvonalaiseen tilaan annosteltu M3G herkisti kivulle, heikensi morfiinin kipua lievittävää tehoa ja lisäsi välittäjäaine Substanssi P:n määrää selkäytimessä. Veri-aivoesteen ulkopuolisten opioidireseptorien esto metyylinaltreksonilla ei estänyt morfiinin kipua lievittävää vaikutusta tai morfiinitoleranssin kehittymistä, kun keskushermostossa vaikuttava naltreksoni esti molemmat vaikutukset. Glymfaattisen järjestelmän aktivointi deksmedetomidiinilla tai HTS:lla lisäsi lukinkalvonalaiseen tilaan annosteltujen morfiinin, oksikodonin ja naloksonin pitoisuuksia keskushermostossa. Selkäytimen lisääntynyt morfiinipitoisuus paransi sen kipua lievittävää tehoa. Tutkimustulokset viittaavat siihen, että keskushermoston pitkittynyt altistuminen M3G:lle herkistää kivulle ja aiheuttaa ristitoleranssin morfiinille. Veri-aivoesteen ulkopuolisilla opioidireseptoreilla taas ei vaikuta olevan merkitystä kivun hoidon tai toleranssin kehittymisen kannalta nosiseptisessa kivussa. Deksmedetomidiinilla tai HTS:lla aktivoitu glymfaattinen aivo-selkäydinnesteen sisäänvirtaus tehostaa lukinkalvonalaiseen tilaan annosteltujen oksikodonin, naloksonin ja morfiinin kulkeutumista aivoihin ja selkäytimeen
