The Outcome of Radiotherapy After Radical Prostatectomy for the Treatment of Prostate Cancer

However, biochemical (BC) recurrence after surgery occurs in about one third of patients. In these cases, radiotherapy (RT) is one of the options with higher potential for cure in the setting of multimodal treatment. The pathological study provides important information about the risk of disease progression and cancer-specific mortality. RT can either be used as an adjuvant therapy (when there are poor prognostic factors in the pathological study of the surgical specimen) either as a salvage therapy (when there is a diagnosis of BC recurrence). The purpose of this study was to evaluate the efficiency of RT after RP in prostate cancer. A retrospective study was conducted, with a follow-up period of approximately 10 years, observing patients who underwent adjuvant or salvage RT after RP for localized prostate cancer.Materials and methods: A retrospective collection data was performed, consulting the clinical records of the patients who underwent adjuvant or salvage RT in 2007 and 2008 at the Radiotherapy Department of the Coimbra Hospital and University Centre, describing patients’ demographic characteristics, the type of surgery, the pathological study results and the follow-up throughout the time. Statistical analysis was performed using the IBM SPSS Statistics® program, version 21, for descriptive inferential analysis. Kaplan-Meier estimates were performed to determine survival curves.Results: 26 patients were included, with a mean age of 61,9±1,08 years, a median of PSA-initial level of 7,6 ng/mL and the interquartile range of 5,63. After RP, a detectable PSA value was obtained in 53,8% with a median of the PSA values of 0,355 ng/mL (interquartile range 0,883). There were 42,3% surgical specimens classified as pT3a and 19,2% as pT3b, without metastatic ganglion invasion in 80,8%. BC recurrence after RP occurred in 58,3% of the cases, with the necessity of the administration of salvage RT. BC recurrence after RT occurred in 55% of the cases submitted to salvage RT and in 50% of those submitted to adjuvant RT. The overall survival rate and the cancer-specific survival at 10 years was of 96,2% and 100%, respectively.Discussion: Most of the patients were middle-aged men with localized prostate cancer at a pT3N0 stage, according to the TNM grading system. Most of the patients submitted to salvage RT presented negative surgical margins. In the group that performed adjuvant RT, positive surgical margins were observed in 66,7%, thus, integrating criteria for adjuvant RT. There was no significant statistically difference between the two groups in terms of incidence of BC recurrence after RT. The overall survival at 10 years was elevated and the result of the cancer-specific survival rate was 100%.Conclusion: Considering the comorbidities related to RT, a thorough assessment should be made in order to select patients who will benefit from the administration of adjuvant RT after RP, without presenting any signs of recurrence of the disease. When characteristics of aggressive illness are identified, RP followed by adjuvant RT is recommended. In all other cases, early RT administration, at the first signs of BC recurrence, it was proven to be equally effective and prevents the adverse effects related to RT in a significant proportion of patients who will not experience BC recurrence in the future.Introdução: A prostatectomia radical (PR) é uma das opções terapêuticas de primeira linha em doentes com carcinoma da próstata (CaP) clinicamente localizado. Contudo, verifica-se recidiva bioquímica (recidiva BQ) em cerca de um terço dos doentes. Nestes casos, a radioterapia (RT) é uma das opções com maior potencial curativo como complemento ao tratamento cirúrgico. O estudo anatomopatológico da peça operatória dá informação útil acerca do risco de progressão da doença e morte relacionada com doença. A RT pode ser utilizada a título adjuvante (quando existem fatores de mau prognóstico no estudo anatomopatológico da peça cirúrgica) ou a título de resgate (quando existe um diagnóstico de recidiva BQ). O objetivo deste trabalho foi a avaliação da eficácia da RT após PR no CaP. Foi realizado um estudo retrospetivo relativo a um período de aproximadamente 10 anos, onde foram observados doentes submetidos a RT adjuvante ou de resgate após realização de PR com intuito curativo por CaP localizado. Materiais e métodos: Foi realizada uma colheita de dados retrospetiva dos registos clínicos dos doentes que realizaram RT adjuvante ou de resgate nos anos de 2007 e 2008 no serviço de Radioterapia do Centro Hospitalar e Universitário de Coimbra, com caracterização demográfica do doente, do tipo de cirurgia, do estudo anatomopatológico da peça operatória e do seguimento do doente. A análise estatística foi feita recurso ao programa IBM SPSS Statistics®, versão 21, com cálculo de estatística descritiva e inferencial. Foram feitas estimativas de Kaplan-Meier para determinar curvas de sobrevivência.Resultados: Foram incluídos 26 doentes, com idade média de 61,9±1,08 anos, com mediana do valor do PSA inicial de 7,6 ng/mL e amplitude interquartil (AIQ) 5,63. Foram classificadas como pT3a 42,3% das peças e como pT3b 19,2%, não existindo invasão ganglionar em 80,8%. A recidiva BQ após PR ocorreu em 58,3% dos casos, com necessidade de administração de RT de resgate. Ocorreu recidiva BQ após RT em 55% dos casos submetidos a RT de resgate e em 50% dos submetidos a RT adjuvante, não se verificando diferença estatisticamente significativa entre as opções terapêuticas. A taxa de sobrevivência global (OS) e de sobrevivência específica de doença a 10 anos foi de 96,2% e 100%, respetivamente.Discussão: A maioria dos doentes eram homens de meia idade, com carcinoma da próstata localizado, em estádio pT3N0, segundo o sistema TNM. Entre os doentes submetidos a RT de resgate, verificou-se que a maioria não tinha invasão de margens cirúrgicas. No grupo que realizou RT adjuvante, 66,7% apresentavam invasão de margens cirúrgicas, sendo esta uma indicação para realizar RT adjuvante. Não se provou uma diferença estatisticamente significativa entre a RT adjuvante e a RT de resgate na ocorrência de recidiva BQ. A OS a 10 anos foi elevada e a taxa de sobrevivência específica de doença a 10 anos foi de 100%.Conclusão: Considerando as comorbilidades associadas ao tratamento com RT deve ser feita uma avaliação rigorosa para selecionar os doentes que beneficiarão da administração de RT após PR sem sinais de recorrência da doença. Perante características de doença agressiva a RT adjuvante à cirurgia está recomendada. Nos restantes casos, a administração de RT precocemente, aos primeiros sinais de recidiva BQ, é igualmente eficaz e evita o risco dos efeitos adversos a curto e a longo prazo associados à RT numa proporção de doentes significativa que não sofrerá recidiva BQ a longo prazo
