Traitement conservateur d’une perforation traumatique de l’oesophage thoracique

Introduction : Esophageal perforation secondary to blunt trauma carries a high morbidity and mortality rates. It is a rare disease. Cervical esophagus is the commonest site of perforation. Numerous options are available for the management of esophageal injuries. The conservative medical management is allowed in well defined cases. Case presentation : We report a rare clinical case of a 40-year- old patient who was injured in a car accident with an impact to the anterior chest. At the time of admission, the patient was alert and complained of chest pain. Clinical findings showed of subacute ischemia in the right upper limb. Chest computed tomography (CT) with intraveinous contrast showed thrombosis of the brachiocephalic artery “a few air bubbles, in the upper mediastinum and a bilateral pneumothorax”. The diagnosis of esophageal perforation was confirmed by a second CT with ingestion of a water-soluble contrast agent. Outcome was favourable on antibiotics and parenteral nutrition. Conclusion : Provided that no clinical signs of sepsis are present and that the patient rapidly improves the conservative management of traumatic perforation of the thoracic oesophagus is a sounded option.Introduction : Les perforations par traumatisme externe de l’oesophage sont associées à une morbi-mortalité élevée. C’est une pathologie rare et fréquemment localisée au niveau de l’oesophage cervical. Le traitement reste controversé en raison de la variété des moyens thérapeutiques disponibles. Le traitement conservateur médical reste admis dans des conditions bien définies. Cas clinique : Nous rapportons un cas clinique rare chez un patient de 40 ans, victime d’un accident de la voie publique (AVP) avec point d’impact thoracique. A l’admission, le patient était conscient, avec des douleurs thoraciques. L’examen clinique trouvait une ischémie subaigüe du membre supérieur droit. A l’angioscanner thoracique on notait la présence d’une thrombose du tronc artériel brachio-céphalique, de quelques bulles d’air au niveau du médiastin supérieur ainsi qu’un pneumothorax bilatéral. La confirmation de la perforation oesophagienne était apportée par un deuxième scanner thoracique avec ingestion d’un produit de contraste hydrosoluble. Sous une alimentation parentérale exclusive et antibiotiques, l’évolution était satisfaisante. Conclusion : En l’absence de tous signes cliniques de sepsis majeur et devant l’amélioration clinique rapide de l’état clinique du patient, le traitement médical des perforations traumatiques de l’oesophage thoracique est une attitude à préconiser
