Introduction : This cancer is composed of a benign glandular component and a sarcomatous stroma. It is characterized by its rarity, difficulties in both diagnostic and therapeutic care. Case report : Patient aged 65 years ald, complaning of a post-menopausal bleeding, the gynecological examination ohowed a collar - like benign polyp, both the ultrasound examination info showed an endometrial hypertrophy and the endometrial biopsy examination were negative. The positive diagnosis was based on the biopsy anatomopathological examination, after a subtotal hystrectomy with bilateral ovarian excision operation. This operation was ended by a neck - isthmectomy. Discussion : Uterine adenosarcoma represents only 8% of all sarcomatous period. Its clinical symptoms are highly polymorpic dominated by the presence of metrorrhagia. Only the anatomo-pathological examination allows for the positive diagnosis by demonstrating the presence of malignant mesenchymal elements and benign epithelial components. Surjery is the main treatment the radio or chemotherapy were instituted in the advanced cases but their usefulness in terms of survival benefet remains controversal. Conclusion : The treatment involves a maltidisciplinary team to estbalish a diagnostic and theraputic prpper conduct especially in advanced cases.Introduction : Il est composé d’un compartiment glandulaire bénin et d’un stroma sarcomateux ; cancer qui est caractérisé par sa rareté, sa difficulté de prise en charge diagnostique et thérapeutique. Observation clinique : Patiente âgée de 65 ans, se plaignant de métrorragies post-ménopausiques, et présentant à l’examen gynécologique un polype accouché par le col d’allure bénin, et à l’échographie une hypertrophie endométriale d’où la réalisation d’une biopsie endométriale se révélant négative. Le diagnostic d’adénosarcome n’a été posé que sur examen anatomopathologique de la pièce opératoire d’hystérectomie subtotale avec annexectomie, le geste chirurgical a été complété par une cervico-isthmectomie. Discussion : Il ne représente que 8% des tumeurs sarcomateuses, il s’agit d’un cancer de la période post-ménopausique. La symptomatologie clinique est très polymorphe dominée par les métrorragies. Seul l’examen anatomopathologique permet le diagnostic positif en mettant en évidence le contingent mésenchymateux malin et épithélial bénin. La chirurgie est la base du traitement, la radio-et chimiothérapie sont institués dans les formes évoluées mais leur utilité en termes de gain de survie reste encore discutée. Conclusion : Sa prise en charge fait appel à une équipe multidisciplinaire afin d’instituer une conduite diagnostique et thérapeutique adéquate, notamment dans les formes évoluées
