LES FISTULES GASTRO-JÉJUNO-COLIQUES PAR ULCÈRE ANASTOMOTIQUES

Objective: Gastro-jejuno-colic fistula caused by recurrent ulcer was a frequent complication of surgery for peptic ulcer. Since the completion of the vagotomy in the surgical treatment of this pathology, gastr-jejuno-colic fistula is becoming increasingly rare but continues to see. We try to study different profiles: clinic, diagnostic, and therapeutic of this complication. Patients and methods : six cases of gastro-jejuno-colic fistula caused by recurrent ulcer were studied retrospectively between 1969 and 2009 . Results : all the patients were operated for duodenal ulcer; the first intervention consisted of gastro-entro-anastomosis alone in three cases. Clinical presentation was dominated by diarrhea, abdomen pain and weight loss. Fistula was demonstrated on barium enema in three patients. All our patients had a one stage resection and repair of the fistula by gastro-jejunal anastomosis in three cases; segmental resection of the colon was done in two cases; postoperative recovery was simple. Conclusion: in spite of varied clinical presentation, gastro-jejuno-colic fistula caused by recurrent ulcer can be readily diagnosed by barium enema. A one stage resection is the procedure of choice.Introduction : Depuis la réalisation de la vagotomie dans le traitement chirurgicale de l’ulcère gastro-duodénale, les fistules gastro-jéjuno-coliques par ulcère anastomotique deviennent de plus en plus rares. Matériels et méthodes : Nous rapportons rétrospectivement six cas de fistules gastro-jéjuno-coliques par ulcère anastomotique qui ont été colligés entre 1969 - 2009. Résultats : Tous nos patients ont été opérés pour ulcère duodénal. La première intervention consistait en une gastro-entro-anastomose isolée dans trois cas. Le tableau clinique était dominé par les diarrhées, les douleurs abdominales et l’amaigrissement. Le diagnostic a été confirmé par le lavement baryté chez trois malades. Tout nos patients ont été opéré en un seul temps, dont trois ont eu une gastrectomie avec anastomose gastro-jéjunale, la colectomie segmentaire avec anastomose termino-terminale était nécessaire chez deux patients. L’évolution était simple chez tous nos patients. Conclusion : Malgré leurs différents aspects cliniques, les fistules gastro-jéjuno-coliques par ulcère anastomotique sont de diagnostic radiologique. Le traitement est chirurgical et la cure en un seul temps est de choix
