LA BAGASSOSE : ALVÉOLITE ALLERGIQUE EXTRINSÈQUE

Introduction : The extrinsic allergic alveolites are interstitial and alveolar pneumopathies of complex immunological mechanisms, related to the inhalation organic chronicles of substances. Case report : We report the observation of a 61 years old, old patient nicotinic chronicle, without particular pathological antecedents, having a concept of professional exposure to the cane with sugar during 40 years. It presents 4 months before its hospitalization a dyspnea of effort of progressive aggravation currently stage IV of the EEC associated with a productive cough mucopurulente, the whole evolving/moving in a context of apyrexie and slimming quantified to 15 kg. The clinical examination finds a patient polypneic with 26c/mn and rails crepitants bilateral with the sounding pleuropulmonaire. Thoracic radiography shows an interstitial syndrome diffuse reticulo-micronodulaire. The thoracic TDM highlights an aspect in honeycomb evoking a pulmonary fibrosis. Washing bronchioloalveolaire objectifies a rate of polynuclear neutrophiles with 6%. Respiratory functional exploration finds a moderate restrictive syndrome with a reduction in the DLCO. Conclusion : In front of these clinical, radiological and functional criteria, the diagnosis of the bagassose was retained. In the light of this observation, we underline the diagnostic difficulties of the alveolite allergic extrinsic and we propose a review of the literature.Introduction : Bagassose alvéolites allergiques extrinsèques sont des pneumopathies interstitielles et alvéolaires de mécanismes immunologiques complexes, liées à l’inhalation chroniques de substances organiques. Observation clinique : Il s’agit d’un patient de 61 ans, tabagique chronique, sans antécédents pathologiques particuliers, ayant une notion d’exposition professionnelle à la canne à sucre pendant 40 ans. Il présente 4 mois avant son hospitalisation une dyspnée d’effort d’aggravation progressive actuellement stade IV de la CEE associée à une toux productive mucopurulente, le tout évoluant dans un contexte d’apyrexie et d’amaigrissement chiffré à 15 kg. L’examen clinique trouve un patient polypneique à 26c/mn et des râles crépitants bilatéraux à l’auscultation pleuropulmonaire. La radiographie thoracique montre un syndrome interstitiel réticulo-micronodulaire diffus. La TDM thoracique met en évidence un aspect en rayon de miel évoquant une fibrose pulmonaire. Le lavage bronchioloalvéolaire objective un taux de polynucléaires neutrophiles à 6%. L’exploration fonctionnelle respiratoire retrouve un syndrome restrictif modéré avec une diminution de la DLCO. Devant ces critères cliniques, radiologiques et fonctionnels, le diagnostic de la bagassose a été retenu. Conclusion : à la lumière de cette observation, nous soulignons les difficultés diagnostiques de l’alvéolite allergique extrinsèque et nous proposons une revue de la littérature
