EPIDÉMIOLOGIE ET RÉSISTANCE AUX ANTIFONGIQUES DES CANDIDA ISOLÉES DANS LES URINES

Introduction: The urinary infections with candida have been in recrudescence for these last years in particular in hospital medium. However, there is no an absolute criteria to differentiate a colonization from a true infection. Our objective is to determine the frequency of the candiduries, their places in these nosocomial urinary infections, as well as the sensitivity of the species isolated to principal antifungal. Materials and methods: This study is carried out at the laboratory of parasitology-mycology of the hospital Cheikh Zaid over one two years period. 2988 patients are included. Results: fifty-seven cases of urinary infection with Candida are diagnosed. The frequency of the urinary infections any germs confused is of 29% and that of the urinary infections with Candida is of 2%. The place of the candidurie in the urinary infections is of 7%. The Candida albicans was insulated among 31 patients (54%) followed by the Candida glabrata (24%), Candida tropicalis (17%) and Candida parapsilosis (7%). Antifongigramme showed resistance to Candida glabrata in 33% of cases, and Candida tropicalis in 20% of cases to fluconazole. Conclusion: The candiduries became increasingly frequent especially in medium of reanimation, of made emergence of new species not albicans. This candidurie can be the witness of a simple colonization, as it can be a sign of alarm of a major infection, primary or secondary with a hematogen dissemination.Introduction : Les candiduries sont en recrudescence depuis ces dernières années en particulier en milieu hospitalier. Pourtant, il n’existe toujours pas de critères absolus pour différencier une colonisation d’une vraie infection. Notre objectif est de déterminer la fréquence des candiduries, leurs places dans ces infections urinaires nosocomiales, ainsi que la sensibilité des espèces isolées aux principaux antifongiques. Matériels et Méthodes : Cette étude est réalisée au laboratoire de parasitologie-mycologie sur une période de deux années avec 2988 patients inclus. Résultats : cinquante sept cas d’infection urinaire à Candida sont diagnostiqués. La fréquence des infections urinaires tout germes confondu est de 29% et celle des infections urinaires à Candida est de 2%. La place de la candidurie dans les infections urinaires est de 7%. Le Candida albicans a été isolé chez 31 patients (54%) suivi du Candida glabrata (24%), Candida tropicalis (17%) et Candida parapsilosis (7%). l’antifongigramme a montré une résistance de Candida glabrata dans 33% des cas, et Candida tropicalis dans 20% des cas au fluconazole. Conclusion : Les candiduries sont devenues de plus en plus fréquentes surtout en milieu de réanimation, du faite de l’émergence de nouvelles espèces non albicans. Cette candidurie peut être le témoin d’une simple colonisation, comme elle peut être un signe d’alarme d’une infection profonde, primaire ou secondaire à une dissémination hématogène
