Tumor del estroma gastrointestinal (GI) reporte de un caso

Given the rare nature of these tumors, the distribution and even the advent of molecular diagnostic criteria for gastrointestinal stromal tumors (GIST) is of interest; although they are the most frequent non-epithelial tumors of the small intestine. GISTs are characterized by indolent clinical symptoms, including vague abdominal pain, weight loss, and occult gastrointestinal bleeding. GISTs are typified by reaching massive size before surgical presentation. Histology and immunohistochemistry are essential for its diagnosis. We present the case of a 55-year-old patient. The clinical diagnosis was of: Inflammatory abdominal syndrome secondary to complicated acute appendicitis appendicular plastron, due to palpable mass in right inferior cuadrant, emergency surgery was decided. During surgery, a mass of stone consistency was identified that encompasses the cecum and terminal ileum about 30 cm. that obstructs intestinal lumen, free purulent fluid in the cavity, it was decided to resect 30 cm of the terminal and meso ileum and defunctionalize the small intestine. The pathology study with Hematoxylin / eosin and immunohischemistry reveals ileal leiomyosarcoma.Dada la naturaleza rara de estos tumores, despierta interés la distribución y hasta el advenimiento de criterios diagnósticos moleculares para tumores del estroma gastrointestinal (GIST); aunque son los tumores no epiteliales más frecuentes del intestino delgado. Los GIST se caracterizan por síntomas clínicos indolentes, incluso dolor abdominal vago, pérdida de peso y sangrado gastrointestinal oculto. Los GIST se tipifican por alcanzar un tamaño masivo antes de la presentación quirúrgica. La histología e inmunohistoquímica son fundamentales para su diagnóstico. Presentamos el caso de una paciente de 55 años. El diagnóstico clínico fue de: Síndrome abdominal inflamatorio secundario a apendicitis aguda complicada plastrón apendicular, por masa palpable en FID, se decide cirugía de urgencia. Durante la cirugía se identifica masa de consistencia pétrea que abarca ciego e íleon terminal 30 cm aprox. que obstruye lumen intestinal, líquido purulento libre en cavidad, se decide resecar 30 cm de íleon terminal y meso y desfuncionalizar intestino delgado. El estudio de Patología con Hematoxilina/eosina e inmunohisquimica revela leiomiosarcoma de íleon
