Oral health-related quality of life in patients with cleft lip and palate

ZobārstniecībaVeselības aprūpeDentistryHealth CareDarba galvenais mērķis bija izpētīt ar mutes dobuma veselību saistīto dzīves kvalitāti ietekmejošos faktorus pacientiem ar iedzimtām lūpu un aukslēju šķeltnēm, izmantojot uz pierādījumiem balstītas zinātniskas publikācijas. Kā hipotēze tika izvirzīts tās, ka rehabilitētu pacientu ar iedzimtām lūpu un aukslēju šķeltnēm dzīves kvalitāte neatšķiras no pārējo iedzīvotāju vai populācijas dzīves kvalitātes un noteiktiem faktoriem: vecumam, dzimumam, šķeltņu fenotipam un ķirurģiskās ārstēšanas esamībai ir ietekme uz dzīves kvalitātes novērtējumu indivīdiem ar rehabilitētām iedzimtām lūpu un aukslēju šķeltnēm. Darbā tika iekļauti 60 pētījumi no 2010. gada līdz 2021. gadam par dzīves kvalitāti pacientiem ar lūpu un aukslēju šķeltnēm. Tika noteiktas dzīves kvalitātes atšķirības pacientiem ar šķeltnēm salīdzinājumā ar populācijas normām, dažādās vecuma grupās, starp dzimumiem, ar dažādiem šķeltņu veidiem, ķirurģiskas ārstēšanas ietekme uz dzīves kvalitāti, kā arī pacientu vecāku dzīves kvalitātes uztveres novērtējums. Darbā ir apskatīti dzīves kvalitātes novērtēšanas instrumenti: OHIP, COHIP, SF-36, ECOHIS, FIS, CPQ, PedsQL, QoLAdoCleft. Tika konstatēts, ka rehabilitētu pacientu ar iedzimtām lūpu un aukslēju šķeltnēm dzīves kvalitāte atšķiras no pārējo iedzīvotāju vai populācijas dzīves kvalitātes; salīdzinot ar populācijas normām, šķeltņu pacientu dzīves kvalitāte ir zemāka; jo smagāks šķeltņu defekts, jo sliktāka dzīves kvalitāte; dzimumam nav ietekmes uz dzīves kvalitāti; vecāki savu bērnu ar šķeltnēm dzīves kvalitāti vērtē kā sliktāku, salīdzinājumā ar pašiem bērniem; ķirurģisku pacientu dzīves kvalitāte ir zemāka, nekā tiem pacientiem, kuriem ķirurģija nebija nepieciešama; pēc ķirurģiskām operācijām šķeltņu pacientu dzīves kvalitāte uzlabojas; no pieejamiem ar mutes veselību saistītās dzīves kvalitātes novērtēšanas instrumentiem vispiemērotākais šķeltņu pacientu dzīves kvalitātes novērtēšanai ir OHIP-14.The main aim of the study was to investigate the factors influencing the quality of life related to oral health in patients with congenital cleft lip and palate using evidence-based scientific publications. As a hypothesis was put forward that the quality of life of rehabilitated patients with congenital cleft lip and palate does not differ from the quality of life of the rest of the population and certain factors: age, gender, type of cleft and surgical treatment have an impact on quality of life assessment in rehabilitated patients with congenital cleft lip and palate. The study includes 60 literature sources from 2010 to 2021 on quality of life in patients with cleft lip and palate. Differences in the quality of life of patients with clefts compared to population norms, in different age groups, between the genders, with different types of clefts, the impact of surgical treatment on the quality of life, as well as the assessment of patients parents' perception of quality of life were identified. The study considers the quality of life assessment instruments: OHIP, COHIP, SF-36, ECOHIS, FIS, CPQ, PedsQL, QoLAdoCleft. The main conclusions were as follows: the quality of life of rehabilitated patients with congenital cleft lip and palate was found to be different compared to population norms; the quality of life of patients with clefts is lower; the more severe the defect, the worse the quality of life; gender has no effect on quality of life; parents rate their child's quality of life as worse than their own children; the quality of life of surgical patients is lower than that of patients who did not require surgery; the quality of life of patients with clefts improves after surgical operations; from the available oral quality of life assessment instruments, OHIP-14 is the most appropriate for assessing the quality of life of patients with clefts
