This project seeks to explore the current local options and resources for community members receiving care at Brookfield Primary Care in Brookfield, Connecticut. Furthermore, this project provided a pamphlet to increase provider and patient awareness about free and low-cost resources within the local community to encourage diabetic patients to increase their activity level.
