Background: Anterior cruciate ligament reconstructions (ACLR) fail at a rate of 10–15%, with graft impingement often a cause. In this study we investigate the prevalence and causes of impingement seen during ACLR surgery. Methods: We reviewed consecutive primary ACLR from 2012-2018. Graft impingement was estimated intraoperatively by placing the arthroscope through the tibial tunnel and passively extending the knee, observing how much was obscured by the lateral femoral condyle from an anterior and lateral direction.
