Involvement of aquaporin-5-interacting protein partners and hyperosmolarity in the physiopathology of Sjögren’s syndrome

Le syndrome de Sjögren (SS) est une maladie auto-immune chronique touchant les glandes exocrines et en particulier les glandes salivaires (SG) et lacrymales (LG) et engendrant un syndrome sec (SICCA). Il a été observé que les symptômes secs pourraient être la conséquence d’un processus complexe dans lequel les cellules lymphocytaires infiltrent les SG qui sont soumises à une stimulation inflammatoire qui entraîne une destruction épithéliale. La sécrétion salivaire est dépendante de la translocation intracellulaire de l’aquaporine 5 (AQP5) depuis des vésicules intracellulaires vers la membrane apicale des cellules acinaires. Les SG de patients atteints du SS présentent une localisation aberrante de l’AQP5 qui se localise dans la région basolatérale des cellules plutôt qu’apicale. Sur base de l’importance des interactions protéine-protéine dans les mécanismes de translocation des protéines, nous avons étudié l'implication des partenaires protéiques de l’AQP5 dans son trafic altéré vers la membrane apicale. Le rôle des protéines Prolactine Inducible Protein (PIP) et Ezrin a été étudié en mettant en évidence la base moléculaire des interactions et leur implication dans le trafic et localisation de l’AQP5 dans les SG. Nos données montrent que l'interaction AQP5-PIP et AQP5-Ezrin est présente dans les SG humaine et de souris et qu’elle apparaît altérée chez les patients SS. L’ensemble de nos résultats suggère que le trafic et la localisation aberrante de l’AQP5 pourraient dépendre d'un complexe composé par plusieurs protéines comprenant PIP et Ezrin. La localisation altérée de l'AQP5 dans les acini des SG de patients atteints du SS et l'apoptose des cellules acineuses pourraient induire une accumulation de NaCl dans la lumière de la glande qui ne serait pas suivie d'un flux d'eau au travers l’AQP5. Ainsi, la génération d'un environnement hyperosmolaire pourrait soumettre les cellules acinaires à un stress hyperosmolaire chronique (HOS) qui aboutirait à une réponse inflammatoire, à une altération de l’architecture glandulaire et à une exacerbation du SS. Par conséquent, nous avons étudié la réponse au stress hyperosmolaire des cellules acinaires. Nous avons montré que HOS augmente les taux d’ARNm de CCL2 via une voie de signalisation intracellulaire impliquant le calcium intracellulaire, PKC, P38, et l'activation des facteurs de transcription CREB, HIF1A et KLF5.Sjögren’s syndrome (SS) is a chronic autoimmune disease affecting exocrine glands such as lacrimal and salivary glands. The most common hallmarks characterizing salivary glands (SGs) are altered SGs architecture, lymphocytic infiltration, and sicca symptoms. It has been postulated that sicca symptoms observed in SS patients could be due to a double step process whereby lymphocyte cells infiltrate the SG which is subjected to inflammatory stimulus and results in epithelial destruction. Recent studies have also observed that Aquaporin-5 (AQP5), a principal player of saliva formation, could be involved in SS SGs altered function. The circulating anti-AQP5 antibodies in sera of SS patients and the altered trafficking of AQP5 could explain the mechanistic process underlying SS SGs dryness. In the present work, we investigate the involvement of AQP5 protein partners in its altered trafficking to the apical membrane of acinar cells. The role of Prolactin-inducible protein (PIP) and Ezrin was studied highlighting the molecular basis of the interactions and their involvement in AQP5 trafficking and localization in SGs. Our data show that, in physiological condition, one molecule of PIP interacts with a AQP5 tetramer. In PIP knockout mice SGs, the absence of PIP is associated with a mislocalization of AQP5 suggesting its role in AQP5 proper trafficking. AQP5-PIP interaction appears altered in SS patients, where PIP is much less expressed compared to the control patients and the localization is basal instead of apical. In the same way, the study of Ezrin as a AQP5 interacting protein allowed us to identify its involvement in AQP5 trafficking. Interestingly, in SS patients, Ezrin and AQP5 interactions were reduced and mislocalized. All together, these results suggest that AQP5 altered trafficking and localization could be dependent on a complex composed by several proteins comprising PIP and Ezrin. In addition, the altered AQP5 localization in SS acini of SGs and the acinar cells apoptosis, could induce an accumulation of NaCl in the lumen of the gland not followed by water flow throw-out AQP5 channel. The generation of a hyperosmotic environment could submit the acinar cells to a chronic stress that would culminate in an inflammatory response, altered gland architecture and SS exacerbation. Therefore, we investigate the hyperosmotic stress responses of acinar cells. We showed that HOS induced CCL2 mRNA levels increase via an intracellular signaling pathway involving intracellular calcium, PKC, P38, the activation of CREB, HIF1A and KLF5 transcription factors and possibly their binding to the CCL2 gene promoter.Doctorat en Sciences biomédicales et pharmaceutiques (Médecine)
