روش های دارویی و غیردارویی موثر در پیشگیری از تهوع و استفراغ در عمل جراحی کوله سیستکتومی ؛ یک مطالعه مروری روایتی و نظام مند

Introduction: Postoperative nausea and vomiting are common and uncomfortable postoperative complications. Different pharmacological and non-pharmacological methods can be used to reduce or control nausea and vomiting after cholecystectomy. The present study investigates the effectiveness of pharmacological and non-pharmacological methods in reducing or controlling nausea and vomiting in cholecystectomy. Methods: In this narrative and systematic review, to find articles in Persian language, SID and Magiran databases, and Google Scholar search engine, from 2000 to 2020, were searched by 2 authors, separately, using Persian keywords equal to cholecystectomy, nausea, and vomiting. Information of the selected articles including: first author, year of publication of the article, title of the study, sample size, drug used, tools studied, and finally, patients’ outcomes were recorded in the data extraction form. The quality of the reviewed articles was evaluated using a checklist related to clinical trial studies. Results: In the initial stage of searching for articles, 558 studies were reviewed. The researchers then reviewed the articles found and 544 studies were eliminated due to not being relevant to the subject under study, duplication, and lack of access to the complete article. Finally, 14 studies were used to write this study. Among the studied articles, metoclopramide was evaluated in 7 studies, ondansetron in 6 studies, dexamethasone in 6 studies, gabapentin in 2 studies, and intraperitoneal bupivacaine, propofol, promethazine, ginger, acupressure, and intravenous crystalloid were examined in one study each. Conclusion: The results of the present study showed that among the pharmacological methods to control nausea and vomiting in patients undergoing cholecystectomy, the first choice of anesthesiologist can be ondansetron. Also, in non-pharmacological methods, acupressure and ginger can both reduce nausea and vomiting after surgery.مقدمه: تهوع و استفراغ بعد از عمل جراحی، از عوارض شایع و ناراحت کننده ی بعد از عمل است. جهت کاهش یا کنترل میزان تهوع و استفراغ بعد از عمل جراحی کوله سیستکتومی می توان از روش های دارویی و غیر دارویی متفاوتی استفاده نمود. مطالعه حاضر به بررسی کارایی روش های دارویی و غیر دارویی در جهت کاهش یا کنترل میزان تهوع و استفراغ در عمل جراحی کوله سیستکتومی می پردازد. روش کار:  در این مطالعه مروری روایتی و نظام مند جهت جستجوی مقالات پایگاه های اطلاعاتی فارسی زبان SID، مگیران و موتور جستجوی گوگل اسکالر از سال 2000 تا 2020 با استفاده از کلید واژه های فارسی؛ کوله سیستکتومی، تهوع، استفراغ، توسط 2 نفر از نویسندگان به صورت مجزا مورد جستجو قرار گرفت. اطلاعات مربوط به مقالات منتخب شامل: نویسنده اول، سال انجام انتشار مقاله، عنوان مطالعه، حجم نمونه، داروی مورد استفاده، ابزار مورد بررسی و در نهایت پیامد بیماران در فرم استخراج داده ها ثبت گردید. ارزیابی کیفیت مقالات مورد بررسی با استفاده از چک لیست مربوط به مطالعات کارآزمایی بالینی انجام شد. یافته ها: در مرحله اولیه جستجوی مطالعات 558 مطالعه بررسی گردید. سپس پژوهشگران به بررسی مقالات جستجو شده پرداختند و تعداد 544 مطالعه بدلیل مرتبط نبودن با موضوع مورد مطالعه و تکراری بودن و عدم دسترسی به مقاله کامل حذف گردید. در نهایت 14 مطالعه جهت نگارش این مطالعه استفاده گردید. از میان مطالعات مورد بررسی، متوکلوپرامید در 7 مطالعه، اندانسترون در 6 مطالعه، دگزامتازون در 6 مطالعه، گاباپنتین در 2 مطالعه و یک مطالعه هم مربوط به هر یک از داروهای بوپیواکایین داخل پریتوئن، پروپوفول، پرومتازین، زنجبیل، طب فشاری و کریستالوئید وریدی مورد بررسی قرار گرفته بود. نتیجه گیری: نتایج مطالعه حاضر نشان داد که در روش های دارویی کنترل تهوع و استفراغ در بیماران تحت عمل جراحی کوله سیستکتومی، انتخاب اول متخصص بیهوشی می تواند داروی اندانسترون باشد. همچنین در روش های غیر دارویی طب فشاری و زنجبیل هر دو می توانند سبب کاهش تهوع و استفراغ بعد از عمل جراحی شوند
