Breaking bad news in the health field: integrative review and clinical-qualitative research with hospital psychologists

This dissertation consists of two complementary studies on the communication of bad news in the context of health care. Study 1 aimed, through an integrative literature review, to map and analyze the national scientific production on the subject. References were located through consultations in the SciELO-Brasil and PEPsic databases. The bibliographic sample consisted of 20 references. In terms of the dimensions of analysis considered, there was a predominance of empirical research, with a qualitative approach, whose objectives – directly or indirectly – refer to protocols for communicating bad news and which were developed with physicians and/or medical students. Furthermore, the findings reported by the references reinforce that the communication of bad news often causes psychological distress both in patients and their families and in health professionals. Study 2, on the other hand, aimed to understand the meanings attributed to the communication of bad news by hospital psychologists. This is a clinical-qualitative study, in which 17 hospital psychologists who worked in sectors dedicated to urgent and emergency care took part. The instrument used was the semi-structured interview and the definition of the number of participants was based on the saturation criterion. Data collection was audio-recorded, and the corpus was composed of the transcription of the material and then submitted to content analysis. Three categories were set up, entitled “The the plate's spinner”, “just between us” and “Second aid”. The first category aggregates representative reports of the meanings attributed by the participants to the difficulties that, for them, characterize situations of communication of bad news. The second category encompasses the participants' point of view regarding the role that would be played by hospital psychologists in solving the expectations and needs of each one involved in communicating bad news. The third category gathers reports related to the meanings attributed by the participants to the role of hospital psychologists in communicating bad news based on the identification of a “before” and an “after” regarding these situations.Dissertação (Mestrado)A presente dissertação é constituída por dois estudos complementares acerca da comunicação de más notícias no contexto da atenção em saúde. O Estudo 1 teve como objetivo, por meio de uma revisão integrativa da literatura, mapear e analisar a produção científica nacional sobre o assunto. A localização das referências se deu por meio de consultas às bases de dados SciELO-Brasil e PEPsic. A amostra bibliográfica foi composta por 20 referências. Em termos das dimensões de análise consideradas, verificou-se a predominância de pesquisas empíricas, de abordagem qualitativa, cujos objetivos – diretamente ou indiretamente – remetem a protocolos de comunicação de más notícias e que foram desenvolvidas junto a médicos e/ou estudantes de Medicina. Ademais, os achados reportados pelas referências reforçam que a comunicação de más notícias frequentemente provoca sofrimento psíquico tanto nos pacientes e seus familiares quanto nos profissionais de saúde. Já o Estudo 2 teve como objetivo compreender significados atribuídos à comunicação de más notícias por psicólogos hospitalares. Trata-se de um estudo clínico-qualitativo, do qual participaram 17 psicólogos hospitalares que atuavam em setores destinados ao atendimento de urgências e emergências. O instrumento utilizado foi a entrevista semidirigida e a definição do número de participantes se deu com base no critério de saturação. A coleta de dados foi gravada em áudio, sendo que o corpus foi composto pela transcrição do material e, então, submetido à análise de conteúdo. Foram configuradas três categorias, intituladas “O equilibrista de pratos”, “Cá entre nós” e “Segundos socorros”. A primeira categoria agrega relatos representativos dos significados atribuídos pelos participantes às dificuldades que, para os mesmos, caracterizam as situações de comunicação de más notícias. A segunda categoria abarca o ponto de vista dos participantes quanto ao papel que competiria aos psicólogos hospitalares no equacionamento das expectativas e das necessidades de cada um dos envolvidos na comunicação de más notícias. A terceira categoria reúne relatos atinentes aos significados atribuídos pelos participantes à atuação dos psicólogos hospitalares na comunicação de más notícias com base na identificação de um “antes” e um “depois” no que tange a essas situações.2023-12-1
