Analysis of patients with cluster headache in the HUMV Headache Unit

RESUMEN : Fundamento: La cefalea es uno de los motivos de consulta más frecuente en los servicios de Neurología, y aunque las cefaleas trigémino-autonómicas (CTA), especialmente la cefalea en racimos (CR), no sea la causa más frecuente, su prevalencia es significativa. La CR es una de las cefaleas más invalidantes que existen, que cursa con dolor unilateral localizado en el área periorbitaria, retroorbitaria o temporal acompañada de síntomas autonómicos ipsilaterales. A fecha de hoy, las series con un número de pacientes importante con CTA, y en concreto con CR, son escasas. Objetivos: Analizar la experiencia con las CTA, y en especial con las CR, en la Unidad de Cefaleas del Servicio de Neurología del HUMV. Pacientes y Métodos: Revisión del archivo histórico del Servicio de Neurología del HUMV desde su creación en el año 1975, identificando todos los pacientes con diagnóstico de CTA y revisando en detalle su historia clínica. Los diagnósticos se recalificaron de acuerdo con los criterios actuales de la ICHD-3. Posteriormente se recogieron las variables de cada paciente y se transcribieron a una base de datos preparada ad hoc. Resultados: Se han recogido un total de 186 pacientes con CTA; 125 eran CR episódicas, 22 CR crónicas, 11 hemicráneas continuas, 7 hemicráneas paroxísticas y 3 SUNCT. La prevalencia teórica de CR estimada en nuestro medio sería de aproximadamente 0,05%. De los 147 pacientes con CR, el 90% eran varones. La edad media actual de los pacientes fue de 61,5 años, la edad media al inicio de los síntomas era de 32,4 años y la edad media al diagnóstico de 39,2 años, con un retraso diagnóstico de 7 años. La edad media de inicio de los síntomas para la CR episódica fue de 31,5 y para la CR crónica de 37,7 años (p=0,018). Los pacientes tenían aproximadamente un racimo de 2 meses (7,7 semanas de media) de duración, cada 3,6 años de media. Hemos encontrado un 2% de pacientes con síntomas típicos de aura coexistiendo con la CR y un 2,7% que refieren antecedentes familiares de CTA. El 75,6% de los pacientes eran fumadores y el 18,1% bebedores. De los pacientes con CR, 61 tenían hecha una TC y 50 tenían hecha una RM; ninguno de los hallazgos de estos estudios tenía correlación clínica con la CR. Habían fallecido 24 pacientes con CR, con una media de edad de 68,6 años. Conclusiones: La CR no es rara: una de cada 2000 personas padece CR en Cantabria. El retraso diagnóstico de 7 años indica el escaso conocimiento de esta enfermedad. Nuestros datos confirman el gran predominio masculino de la enfermedad y establecen una media de un racimo de aproximadamente 2 meses de duración cada 3-4 años. Ser mujer y un inicio tardío de la clínica se asoció a padecer una CR crónica. No es rentable llevar a cabo estudios de RM en pacientes con CR que cumplan criterios ICHD-3. Por último, los datos encontrados sugieren que la CR se asocia a una mayor mortalidad, probablemente en relación con la alta tasa de hábitos de vida poco tolerables.ABSTRACT : Background: Headache is one of the most frequent reasons for consultation in Neurology services, and although Trigeminal-autonomic cephalalgias (TACs), especially cluster headache (CH), are not the most frequent cause, their prevalence is significant. CH is one of the most disabling existing headaches, which presents with unilateral pain located in the periorbital, retroorbital or temporal region accompanied by ipsilateral autonomic symptoms. To date, the series with a significant number of patients with TACs, and specifically with CH, are scarce. Objectives: To analyze the experience with TACs, and especially with CH, in the Headache Unit of the HUMV Neurology Service. Patients and Methods: Review of the historical archive of the HUMV Neurology Service since its creation in 1975, identifying all patients with a diagnosis of TACs and reviewing their clinical history in detail. Diagnoses were re-rated according to current ICHD-3 criteria. Subsequently, the variables of each patient were collected and transcribed into a database prepared ad hoc. Results: A total of 186 patients with TACs were collected; 125 were episodic CH 22 chronic CH, 11 continuous hemicrania, 7 paroxysmal hemicrania, and 3 SUNCT. The theoretical estimated prevalence of CH in our setting would be approximately 0.05%. Of the 147 patients with CH, 90% were male. The current mean age of the patients was 61.5 years, the mean age at the onset of symptoms was 32.4 years and the mean age at diagnosis was 39.2 years, with a diagnostic delay of 7 years. The mean age of symptom onset was 31.5 for episodic CH and 37.7 for chronic CH (p = 0.018). The patients had a cluster of approximately 2 months (7.7 weeks on average) long, every 3.6 years on average. We have found 2% of patients with typical symptoms of aura coexisting with CH and 2.7% who report a family history of TACs. 75.6% of the patients were smokers and 18.1% excessive alcohol consumers. Of the patients with CH, 61 had a CT scan and 50 had an MRI; none of the findings from these studies had a clinical correlation with CH. 24 patients with CH had died, with a mean age of 68.6 years. Conclusions: CH is not rare: one in every 2000 people suffer CH in Cantabria. The 7-year diagnostic delay indicates a little knowledge of this disease. Our data confirm the great male predominance of the disease and establish an average of a cluster lasting approximately 2 months every 3-4 years. Being a woman and a late onset of the clinic was associated with suffering from chronic CH. It is not cost-effective to carry out MRI studies in patients with CH who meet ICHD-3 criteria. Finally, the data found suggest that CH is associated with higher mortality, probably in relation to the high rate of poorly tolerable lifestyle habits.Grado en Medicin
