Evaluation of the risk of tuberculosis reactivation in patients using immunobiological in services of reference in Rheumatology in the state of Ceará

Rheumatic diseases, rheumatoid arthritis, and spondyloarthritis are autoimmune, chronic and marginal pathologies. These diseases may be peripheral or axial joints (spinal column). When untreated, individual carriers may develop musculoskeletal sequences and, in some cases, extra-articular manifestations. Pharmacological treatment can be done with NSAIDs, synthetic and biological DMARDs, depending on the disease and indications for each stage of the disease. Biological DMARDs are represented by anti-TNF, anti-IL-6, co-stimulation modulators and anti-CD20 immunobiological agents. The use of drugs that modulate the immune system invariably results in a greater risk for the development of infections. Surgery the behavior in behavior of immunobiológicos, deserves special emphasis in the growth of tuberculosis. This is particularly the case for those using anti-TNF, with a latent reactivation of latent tuberculosis among patients. In this context, the present study evaluated the epidemiological profile of 173 patients with rheumatic diseases, observing that the female gender is the most prevalent; an average of diagnoses of rheumatic disease at the age of 52 years; the most frequent BMI is an overweight class; an average of anti-TNF use was 33.91 months, with adalimumab being the most used (52.02%). As for latent tuberculosis, the strongest protocol for the diagnosis of ILTB was a previous indicator of the case with M. tuberculosis. Of the 173 patients treated, 39 were diagnosed with ILTB, 4 of whom developed TB, 2 of whom were treated for ILTB, 1 were not treated and 1 was not informed.As doenças reumáticas, como artrite reumatoide e espondiloartrite são patologias autoimunes, crônicas e de caráter inflamatório. Essas doenças podem acometer articulações periféricas (pequenas e grandes) ou axiais (coluna vertebral). Quando não tratadas, os indivíduos portadores podem desenvolver sequelas osteomusculares e em alguns casos manifestações extraarticulares. O tratamento farmacológico pode ser feito com AINES, DMARDs sintéticos e biológicos, a depender da doença relacionada e das indicações para cada estágio da doença. Os DMCD biológicos são representados pelos imunobiológicos anti-TNF, anti-IL-6, moduladores de coestimulação e anti-CD20. O uso de medicamentos que modulam o sistema imunológico resulta, invariavelmente, em maior risco para o desenvolvimento de infecções. Dentro desse universo de infecções em usuários de imunobiológicos, merece especial destaque o aumento na incidência de Tuberculose . Esse achado é, particularmente, relevante para aqueles que usam anti-TNF, sendo a reativação de Tuberculose Latente a principal infecção oportunista nesses pacientes. Nesse contexto, o presente estudo avaliou o perfil epidemiológico de 173 pacientes portadores de doenças reumáticas, observando que o sexo feminino é o mais prevalente; a idade média do diagnóstico de doenças reumáticas desses pacientes é de 52 anos; o IMC mais frequente é a classe de sobrepeso; a média de uso de anti –TNF foi de 33,91 meses, sendo o adalimumabe o mais utilizado (52,02%). Quanto a Tuberculose Latente, o critério mais forte para diagnóstico de ILTB foi a presença de contato prévio com indivíduos infectados pelo Mycobacterium tuberculosis (MTB). Dos 173 paciente analisados, 39 receberam o diagnóstico de ILTB, sendo que 4 desenvolveram TB, sendo que 2 realizaram tratamento para ILTB, 1 não realizou e 1 não foi informado
