Differences in CR1 / CD35 complement receptor expression in red blood cells and leukocytes in patients with visceral leishmaniasis before and after treatment

Visceral leishmaniasis or Kala-azar is a chronic systemic disease, endemic in northeastern Brazil, caused by the parasite Leishmania infantum chagasi and transmitted by the bite of the sand fly Lutzomyia longipalpis. Laboratory findings of pancytopenia are associated with polyclonal activation of B cells, with high levels of circulating immune complexes (CIC). The receptors for complement type 1 (CR1/CD35) in erythrocytes (CR1/E) and in neutrophils (CR1/N) have the function of CIC clearance. The expression of CR1 is modulated during diseases and tends to normalize after a period of remission. Reducing the CR1/E was observed in autoimmune diseases and chronic infectious diseases, being correlated with increased levels of CIC and the severity of symptoms. It has also been observed a reduction in the CR1/N in autoimmune diseases. This study aimed to investigate in patients with Kala-azar, before and after treatment, the expression of CR1 on erythrocytes and leukocytes by flow cytometry and measuring circulating immune complexesby enzyme immunoassay. At the moment of diagnosis, patients compared with the healthy control group showed a reduction in the expression of CR1/E, as well as the frequency of erythrocytesCD35+. There was also a reduction in the expression of CR1/N without reducing the frequency of neutrophilsCD35+, while in lymphocytes and monocytes there was an increase in CR1 expression. After 24 days of treatment, the expression of CR1/E and CR1/N increased significantly, showing a tendency to recover after treatment. CR1expression in monocyte decreased and CR1expression in lymphocyte showed a tendency to normalization. Before treatment, 66% of patients, CIC levels were approximately nine times higher and no correlation with the expression of CR1/E. After treatment, a reduction in CICwas observedbut the levels yet still remaining above the normal. The results suggest that differences in expression of the CR1 in erythrocytes and leucocytesmay be considered a phenomenon acquired with pathology, but no correlation with the levels of circulating immune complexes.A Leishmaniose Visceral ou Calazar é uma doença crônica sistêmica, endêmica na região nordeste do Brasil, causada pelo protozoário parasita Leishmania infantum chagasi e transmitida pela picada do flebotomíneo Lutzomyia longipalpis.Ao quadro de pancitopenia associa-se a ativação policlonal de linfócitos B, com altos níveis de imunocomplexos circulantes (ICC). O receptor para complemento tipo 1 (CR1/CD35) nos eritrócitos (CR1/E) e nos neutrófilos (CR1/N) faz a retirada de ICC. A expressão do CR1 nas células é modulada durante as doenças e tende a se normalizar após um período de remissão. A redução do CR1/E foi observada em doenças autoimunes e em patologias infecciosas crônicas, sendo correlacionada com níveis aumentados de ICC e com a gravidade do quadro clínico. Também já foi observada a redução do CR1/N em doenças autoimunes. Este estudo teve como objetivo investigar em indivíduos com Calazar, antes e após o tratamento, a expressão do CR1 em eritrócitos e leucócitos, por citometria de fluxo e dosar a quantidade de ICC, por ensaio imunoenzimático. No momento do diagnóstico, os pacientes, quando comparados com o grupo controle saudável, apresentaram uma redução na expressão do CR1/E, assim como na frequência de eritrócitos CD35+. Também ocorreu redução na expressão do CR1/N, sem redução da frequência de neutrófilosCD35+, enquantonos linfócitos e monócitos ocorreu um aumento da expressão do CR1. Após o tempo médio de 24 dias de tratamento, as expressões do CR1/E e do CR1/N aumentaram significativamente, mostrando uma tendência à recuperação após o tratamento. A expressão do CR1 dos monócitos diminuiu e, a dos linfócitos mostrou uma tendência à normalização em relação aos controles. Antes do tratamento,em 66% dos pacientes, os níveis de ICC estavam aproximadamente nove vezes mais elevados e sem correlação com os valores de CR1/E. Após o tratamento, foi observada uma redução nos níveis de ICC nesses pacientes, contudo ainda mantendo-se acima do normal.Os resultados sugerem que as diferenças na expressão do número de CR1 nas populações celulares investigadas podem ser consideradas um fenômeno adquirido com a patologia, contudo sem correlação com os níveis de imunocomplexos circulantes
