Association between body composition and quality of life in patients with chronic kidney disease undergoing hemodialysis

The literature indicates that low quality of life (QOL) scores are predictors of mortality in individuals undergoing hemodialysis (HD). The QOL of individuals with CKD is directly associated with modifiable factors (among which anemia, nutritional status) and non-modifiable factors (age, gender). The detection and understanding of the influence of inadequate nutritional status (causing loss of strength and muscle mass) on the QOL of patients with CKD submitted to HD is of fundamental importance to assist in the development of prevention and intervention actions appropriate to the needs of these patients (rehabilitation nutritional, functional, psychological and family). Objective: To define a cutoff point for lean tissue to classify low muscle mass in end stage kidney disease in patients undergoing HD, and based on this cut-off point compare the QOL level between patients with normal muscle mass and low muscle mass. Methods: We studied 245 patients with end-stage renal disease in HD at the two dialysis centers located in the north region of Ceará state, Brazil, in April 2017. Demographic, clinical and laboratory data were collected. Patients’ QOL was assessed by the brazilian version of the Medical Outcomes Study 36-Item Short Form Health Questionnaire (SF-36). Bioimpedance analysis and handgrip testing were used, respectively, to evaluate muscle mass and muscle strength. Low muscle mass was classified according to muscle strength, and the QOL dimensions were compared between patients with normal and low muscle mass. The dimensions that differed in the comparison were analyzed, along with muscle mass, as dependent variables in a linear regression model to detect independent predictors of QOL scores. Results and Discussion: According to a cut-off point ≤ 15 kg / m² of lean tissue index to classify low muscle mass, a prevalence of 41.2% of low muscle mass was found. Patients with low muscle mass had lower scores related to physical functioning, vitality and mental health. In the multivariate linear regression, the low muscle mass could only predict independently the score related to physical functioning. Conclusion: Low muscle mass affects the physical functioning of patients with end-stage renal disease submitted to HD. It was thought that resistance exercises should be tried among patients in HD with the objective of improving the quality of life.Introdução: A doença renal crônica (DRC) é uma desordem heterogênica que afeta a estrutura e a função dos rins. A literatura indica que baixos escores de qualidade de vida (QV) são preditores de mortalidade em indivíduos submetidos à hemodiálise (HD). A QV dos indivíduos com DRC associa-se diretamente aos fatores modificáveis (dentre os quais anemia, estado nutricional) e não modificáveis (idade, gênero). A detecção e compreensão da influência do estado nutricional inadequado (causando perda de força e massa muscular) sobre a QV dos pacientes com DRC submetidos à HD é de fundamental importância para auxiliar no desenvolvimento de ações de prevenção e intervenção adequadas às necessidades destes pacientes (reabilitação nutricional, funcional, psicológica e familiar). Objetivo: Definir um ponto de corte de tecido magro para classificar baixa massa muscular na doença renal em estágio terminal em pacientes submetidos à HD, e com base neste ponto de corte comparar o nível de QV entre pacientes com massa muscular normal e baixa massa muscular. Métodos: Foram estudados 245 pacientes com insuficiência renal terminal em HD nos dois centros de diálise localizados na região norte do estado do Ceará, Brasil, em abril de 2017. Dados demográficos, clínicos e laboratoriais foram coletados. A QV dos pacientes foi avaliada pela versão brasileira do Medical Outcomes Study 36-Item Short Form Health Questionnaire (SF-36). Bioimpedância e teste de preensão manual, foram, respectivamente, utilizados para avaliar a massa muscular e a força muscular. Classificou-se baixa massa muscular de acordo com a força muscular, e foram comparadas as dimensões de QV entre os pacientes com massa muscular normal e baixa. As dimensões que diferiram na comparação foram analisadas, juntamente com a massa muscular, como variáveis dependentes em um modelo de regressão linear para detectar preditores independentes de escores de QV. Resultados e Discussão: De acordo com um ponto de corte ≤ a 15 kg/m² de índice de tecido magro para classificar a massa muscular baixa, foi encontrada uma prevalência de 41,2% da massa muscular baixa. Os pacientes com baixa massa muscular apresentaram pontuações mais baixas relacionadas com o funcionamento físico, vitalidade e saúde mental. Na regressão linear multivariada, a baixa massa muscular só conseguiu prever de forma independente a pontuação relacionada ao funcionamento físico. Conclusão: A baixa massa muscular afeta no funcionamento físico de pacientes com insuficiência renal terminal submetidos a HD. Pensou-se que exercícios de resistência devem ser experimentados entre os pacientes em HD com o objetivo de melhorar a qualidade de vida
