Socio-epidemiological, clinical and genotypage study of human papilomavirus in women with HIV / AIDS

This study determined the cases of Human Papillomavirus (HPV) among women living with HIV / AIDS, as well as investigated the HPV genotypes present in the cervical mucosa of these women. A cross-sectional, descriptive study developed at the Meireles School Health Center with 50 women living with HIV / AIDS treated at the center, as well as those from other services to perform the uterine cervical cancer screening test. The data were collected through an interview that investigated epidemiological variables, sexual behavior and gynecological prevention, ectocervical and endocervical cytopathological data collection for the evaluation of Papanicolau and Molecular Test of Polymerase Chain Reaction (PCR) in order to identify fragments of viral DNA in the Nucleus and / or cytoplasm of infected cells. Descriptive analyzes were used for the analyzes through the frequency distributions (uni and bivariate) and descriptive measures (means, standard deviation and / or medians). To identify the factors associated with changes in precursor lesions of the cervix, tables of association between the dependent variable in question and the other independent variables were elaborated. Associations were checked by chi-square or Fisher exact test, when necessary. Odds Ratio (OR) and respective confidence intervals (95% CI) were determined to expose the strength of the association with the outcome. In all tests, a significance level of 0.05 (5%) was adopted, and a P value <0.05 was considered statistically significant. In the results, the majority of the women were aged ≥ 40 years (62.0%), average of eight years of study (62.0%), browns (52.0%), unemployed (74.0%), Of a minimum wage (74.0%), did not live with their partner (54.0%) and had children (78.0%). There was a predominance of the category of sexual exposure (92.0%) as a mode of HIV acquisition, heterosexual (92.0%), HIV diagnosis time in five years (46.0%), CD4 + (74.0%) and viral load lower than 50 copies / ml (94.0%) and the use of licit drugs such as alcohol and tobacco (66.0%). About preventive measures against cervical cancer most performed the exam at least once throughout life and the minority were immunized against HPV. Cytopathological results indicated that a significant percentage (66.0%) presented a positive result for the schiller test and some type of inflammation between mild, moderate and marked (90.0%). Three women (6.0%) had HPV test results and low grade intraepithelial lesion and one (2.0%) high grade intraepithelial lesion. There was a statistically significant association between the age of Coitarca and history of STI and between atypias of squamous cells and the use of condoms. Of the 50 women with HIV / AIDS who composed the sample, 44 were tested for HPV, identifying a negative HPV result in 30 (68.0%) women and a positive result in 14 (32.0%). Of the 14 women with a positive HPV test, 12 (86.0%) had subgroup A types considered high oncogenic risk, one (7.0%) presented only type 16 and one (7.0%) presented Type 16 and associated subgroup A. Four (100%) women identified with cervical intraepithelial lesions (ILE) also had HPV, three (6.0%) of them with low-grade cervical LV and high-risk HPV and one (2.0%) with High-grade cervical LIE and HPV 16. It is concluded that women with HIV are vulnerable to cytological alterations, among them cervical intraepithelial lesions and because they present HPV and among them oncogenic subtypes, it is demonstrated that women living with HIV / AIDS of the sample studied are at risk for the development of cervical cancer.Esta pesquisa determinou os casos de Papilomavírus Humano (HPV) entre mulheres que vivem com HIV/aids, bem como investigou os genótipos de HPV presentes na mucosa cervical destas mulheres. Estudo transversal, descritivo desenvolvido no Centro de Saúde Escola Meireles com 50 mulheres que vivem com o HIV/aids atendidas no referido centro, bem como aquelas advindas de outros serviços para realização do exame de prevenção de câncer cérvico uterino. Os dados foram coletados mediante entrevista que investigou variáveis epidemiológicas, de comportamento sexual e prevenção ginecológica, coleta Citopatológica do raspado ectocervical e endocervical para avaliação de Papanicolau e Teste molecular de Reação Cadeia Polimerase (PCR) com o intuito de identificar fragmentos de DNA-viral no núcleo e/ou citoplasma de células infectadas. Para análises utilizaram-se análises descritivas mediante as distribuições de frequências (uni e bivariada) e medidas descritivas (médias, desvio padrão e/ou medianas). Para identificar os fatores associados às alterações das lesões precursoras do colo uterino, foram elaboradas tabelas de associação entre a variável dependente em questão e as demais variáveis independentes. As associações foram verificadas por meio do teste qui-quadrado ou exato de Fisher, quando necessário. Foram determinadas as Odds Ratio (OR) e respectivos intervalos de confiança (IC95%) para expor a força da associação com o desfecho. Em todos os testes foi adotado o nível de significância de 0,05 (5%), sendo considerado estatisticamente significante um valor P < 0,05. Nos resultados a maioria das mulheres tinham idade ≥ 40 anos (62,0%), média de oito anos de estudo (62,0%), pardas (52,0%), desempregadas (74,0%), renda média familiar de um salário mínimo (74,0%), não viviam com o companheiro (54,0%) e possuíam filhos (78,0%). Houve predomínio da categoria de exposição sexual (92,0%) como modo de aquisição do HIV, heterossexuais (92,0%), tempo de diagnóstico do HIV em média de cinco anos (46,0%), contagem de linfócitos T CD4+ superior a 500 células/mm³ (74,0%) e carga viral inferior a 50 cópias/ml (94,0%) e o uso de drogas lícitas como álcool e tabaco (66,0%). Acerca de medidas preventivas contra o câncer de colo do útero a maioria realizou o exame pelo menos uma vez ao longo da vida e a minoria foram imunizadas contra o HPV. Os resultados citopatológicos apontaram que uma parcela significativa (66,0%) apresentou resultado positivo para o teste de schiller e algum tipo de inflamação entre leve, moderada e acentuada (90,0%). Três mulheres (6,0%) tiveram resultados de exames indicando HPV e lesão intraepitelial de baixo grau e um (2,0%) lesão intraepitelial de alto grau. Houve associação estatisticamente significante entre a idade da Coitarca e histórico de IST e entre atipias de células escamosas e o uso de preservativo. Das 50 mulheres com HIV/aids que compuseram a amostra, 44 foram testadas para o HPV, identificando-se resultado negativo de HPV em 30 (68,0%) mulheres e positivo em 14 (32,0%). Das 14 mulheres com teste positivo para o HPV, 12 (86,0%) apresentaram os tipos do subgrupo A considerados de alto risco oncogênico, uma (7,0%) apresentou apenas o tipo 16 e uma (7,0%) o tipo 16 e o subgrupo A associados. Quatro (100%) mulheres identificadas com Lesões Intraepiteliais (LIE) cervical, também apresentavam o HPV, sendo três (6,0%) delas com resultado LIE cervical de baixo grau e HPV de alto risco e uma (2,0%) com LIE cervical de alto grau e HPV 16. Conclui-se que as mulheres com HIV são vulneráveis a alterações citológicas, dentre estas as Lesões Intraepiteliais cervicais, e por apresentarem HPV e entre eles subtipos oncogênicos, demonstra-se que as mulheres vivendo com HIV/aids da amostra estudada possuem risco para o desenvolvimento do câncer do colo do útero
