Topical application of S- nitrosoglutathione gel matrix mucoaderente modulates inflammatory markers in experimental periodontitis in rats

Periodontal diseases, highly prevalent disease in worldwide population, manifest primarily in two distinct entities: plaque-induced gingivitis and periodontitis. In this study, we evaluated the effect of a formulation containing nitric oxide donor, S-nitrosoglutathione (GSNO), in mucoaderente matrix (HPMC) in experimental periodontal disease (EPD). This mucoaderente formulation allows topical application and slow diffusion of GSNO. DPE was induced by insertion of a 3.0 nylon suture around the left upper second molar. Animals was divided in groups: naive, non-treated group, and groups treated with HPMC or GSNO (0.5, 2 and 10 mM). To evaluate the GSNO effects in DPE, we measured: the rate of alveolar bone loss, serum bone alkaline phosphatase, histopathology scores, cytokine assay, immunohistochemistry for iNOS, RANK, RANKL, OPG and TRAP, nitrite and nitrate levels, glutathione (GSH) levels and malondialdehyde (MDA) levels. Gene expression TNF-α, RANK, RANK-L and OPG was measured by qPCR. The GSNO reduced the bone loss, increased bone alkaline phosphatase, decreased the inflammatory cell infiltrate, decreased the release of proinflammatory cytokines (IL-1β and TNF-α and IL-10), diminish gene expression of TNF-α, RANK and RANL/OPG and the iNOS, RANK and RANK-L protein expression, increasing OPG protein expression. Moreover, GSNO reduced oxidative stress, increasing GSH and decreasing MDA and NOx levels. In conclusion, GSNO in mucoaderente formulation promise to improve periodontal disease treatment. However, clinical trials are required to evaluate effect and toxicity GSNO.As doenças periodontais possuem alta prevalência na população mundial e manifestam-se, principalmente, em duas entidades distintas, a saber: a gengivite induzida pela placa bacteriana e a periodontite. O objetivo deste estudo foi avaliar o efeito de uma formulação contendo um doador de óxido nítrico, o S-nitrosoglutationa (GSNO), em matriz mucoaderente (HPMC) que permite a aplicação tópica e difusão lenta do GSNO na doença periodontal experimental (DPE). A DPE foi induzida por inserção de um fio de náilon 3.0 ao redor do segundo molar superior esquerdo, sendo utilizados os seguintes grupos experimentais: Grupo naive, Grupo tratado com HPMC e Grupos tratados com GSNO em diferentes concentrações (0,5, 2 ou 10 mM). Em seguida foram avaliados os seguintes parâmetros: análise do índice de perda óssea alveolar, dosagem de fosfatase alcalina óssea, análise histopatológica, dosagem de citocinas TNF-α e IL-1β, IL-10, imunohistoquímica para iNOS, RANK, RANK-L, OPG e TRAP dosagem de nitrito e nitrato, determinação dos níveis de glutationa reduzida (GSH) e determinação dos níveis de malondialdeído (MDA) e qPCR para TNF-α, RANK, RANK-L/OPG. O GSNO reduziu a perda óssea, aumentou a fosfatase alcalina óssea, reduziu o infiltrado inflamatório, a produção de citocinas pró-inflamatórias IL-1β e TNF-α, reduziu a expressão gênica de TNF-α, RANK e RANKL/OPG e a expressão proteica de iNOS, RANK e RANK-L,TRAP e aumentando a de OPG. Além disso, GSNO reduziu o estresse oxidativo, aumentando GSH e diminuindo MDA e NOx. Concluímos que o GSNO em gel com matriz mucoaderente é promissor para o tratamento da doença periodontal, porém estudos clínicos são necessários para avaliar seu efeito e sua toxidade
