Maturação in vitro de oócitos humanos e posterior fertilização

O objetivo do presente trabalho foi coletar os oócitos ainda imaturos, maturá-los in vitro, fertilizá-los e transferi-los almejando gravidez. As pacientes foram recrutadas no Setor de Reprodução Humana do Hospital das Clínicas e nos ambulatórios de infertilidade do Posto de Atendimento Médico (PAM) da Sagrada Família. Todas assinaram o termo de consentimento livre e esclarecido antes de iniciar o estudo. O protocolo foi aprovado pelo colegiado do curso de pós-graduação em Ginecologia e Obstetrícia. As pacientes selecionadas apresentaram idade entre 18 e 32 anos incompletos e receberam 300UI de hormônio folículo estimulante (FSH) recombinante por via intra-muscular no segundo dia do ciclo e doses adicionais de 150UI no quarto e no sexto dia do ciclo. A coleta ovular deu-se invariavelmente no sétimo dia do ciclo. Os oócitos identificados como imaturos foram colocados em meio TCM 199 acrescido de antibióticos, piruvato, FSH, gonadotrofina coriônica humana (hCG) e soro (serum substitute supplement Irvine Scientific®). Após 48h de cultivo, os oócitos que atingiram o estágio de metáfase II (MII) foram inseminados e os fertilizados transferidos. Foram puncionados 144 folículos com a coleta de 67 oócitos imaturos (46,5%). Atingiram o estágio de metáfase II 43 (64,2 %) e foram inseminados. Destes, 30 fertilizaram e 25 embriões foram transferidos para 10 pacientes. Houve uma gravidez com nascimento de um bebê. Concluiu-se que a técnica de maturar oócitos humanos in vitro previamente à fertilização in vitro é uma técnica totalmente exeqüível, capaz de gerar gravidez.A prospective non randomized, descriptive study was carried out, during the period of November of 1999 until March of 2001, with 20 cycles of in vitro fertilization (IVF) of 15 patients with tubal infertility. The aim of this study was to retrieve immature oocytes for in vitro maturation, in vitro fertilization and embryo transfer in order to achieve pregnancy. The patients were recruited at the Human reproduction Center of Hospital das Clínicas and at out patients department of infertility from Posto de Atendimento Médico (PAM) Sagrada Família. All of them have signed the written consent before the studys beginning. The protocol was approved by board of Gynecology and Obstetrics. The selected patients were at least 18 and at most 32 years of age not completed and received 300UI of follicle stimulating hormone (FSH) recombinant by intramuscular injection at second day of the cycle and additional doses of 150UI at fourth and sixth day of cycle. The oocyte retrieval was performed always at seventh day of the cycle. Those oocytes classified as immatures were cultured in TCM-199 medium with antibiotics, pyruvate, FSH, human corionic gonadotropin (hCG) and serum (serum substitute supplement Irvine Scientific®). After 48 hours of culture oocytes that achieved metaphase II (MII) stage were inseminated, and the fertilized ones transfered. 144 follicles were aspirated, 67 immature oocytes were recovered, 43 achieved the MII stage and were inseminated. 30 fertilized and 25 embryos were transfered to 10 patients. There was one pregnancy with a baby born. We conclude that to mature in vitro human oocytes before in vitro fertilization is a procedure able to achieve pregnancy
