Preditores neuropsicológicos de eficácia do treinamento de pais em grupo para crianças com transtornos externalizantes

Dentre os tratamentos mais eficazes para essas crianças encontram-se os Programas de Treinamento de Pais. Estes visam capacitar os pais a modificarem suas práticas disciplinares e educativas e a atuarem de modo efetivo sobre o padrão comportamental indesejado de seus filhos. Objetivo: Verificar a eficácia do Treinamento de Pais em grupo no tratamento de crianças com Transtornos Externalizantes em uma amostra de crianças provenientes do Ambulatório de Psiquiatria da Infância e Adolescência do HC-UFMG e identificar possíveis fatores neuropsicológicos, clínicos e demográficos preditores da Eficácia do Treinamento de Pais. Método: Vinte e cinco pais de crianças de 6 a 10 anos com Transtornos Externalizantes participaram do Treinamento de Pais e preencheram as escalas antes (T1) e depois (T2) do tratamento. Vinte e um pais provenientes do mesmo ambulatório, que se encontravam na fila de espera para o tratamento, preencheram escalas comportamentais em dois tempos (T1, T2), com intervalo de três meses, correspondente à duração do tratamento. Foi utilizado o teste Wilcoxon (intra-grupos) para analisar a eficácia do tratamento. As crianças e os pais do grupo clínico passaram por avaliação neuropsicológica antes de iniciarem o tratamento. Foi utilizada a correlação de Spearman para verificar a relação entre as variáveis neuropsicológicas, clínicas e demográficas e a taxa de mudança. Foram realizadas regressões lineares com o objetivo de identificar os preditores de eficácia. Resultados: O grupo tratamento apresentou diferença significativa entre os escores em T1 (antes do tratamento) e T2 ( depois do tratamento) ou seja, houve melhora significativa na intensidade de sintomas de desatenção, hiperatividade/impulsividade, sintomas do TDO, comportamentos de desobediência e a severidade desta desobediência assim como nos comportamentos de violação de regras e comportamentos agressivos. O grupo lista de espera não apresentou diferença significativa entre os escores em T1 e T2. Uma maior taxa de melhora nos sintomas de desatenção das crianças foi previsto pela intensidade destes sintomas antes da intervenção e por dificuldades atencionais das crianças na tarefa neuropsicológica. Uma maior taxa de melhora nos sintomas de hiperatividade das crianças foi previsto por dificuldades atencionais das crianças na tarefa neuropsicológica. Uma maior taxa de melhora de sintomas de TDO nas crianças foi prevista pela intensidade desses sintomas e pela dificuldade dos pais na tarefa de flexibilidade cognitiva. Conclusão: A eficácia do tratamento nesta amostra foi comprovada. O pior desempenho dos pais e das crianças nas tarefas neuropsicológicas e uma maior intensidade de sintomas antes do Treinamento de Pais estão associados a uma maior taxa de melhora dos sintomas de desatenção, hiperatividade e TDO nas crianças.Introduction: The Attention Deficit Hyperactivity Disorder (ADHD), Conduct Disorder and Oppositional Defiant Disorder (ODD) are Externalizing Disorders that cause impairments in social, academic and family functioning. Among the most effective treatments for these children are the Parent Training programs. These are designed to enable parents to modify their disciplinary and education practices and enable them to act effectively on the undesired behaviors. Objective: Verify the effectiveness of the Group Parent Training in the treatment of children with Externalizing Disorders in a sample of children from the Clinic of Child and Adolescent Psychiatry, HC-UFMG and identify possibles neuropsychological, clinical and demographic predictors of Parent Training effectiveness. Method: Twenty-five parents of children aged 6 to 10 years with Externalizing Disorders participated in the Parent Training and filled the scales before (T1) and after (T2) treatment. Twenty parents from the same clinic, who were on the waiting list for treatment, filled the behavioral scales in two times (T1, T2), with an interval of three months, corresponding to the duration of treatment. Wilcoxon test was used to analyze the effectiveness of treatment. Children and parents in the clinical group passed through neuropsychological assessment before starting treatment. Spearman correlation was used to investigate the relationship between rate of change and neuropsychological variables, clinical and demographic. Linear regression was used in order to identify predictors of efficacy. Results: The treatment group showed a significant difference between scores at T1 (before treatment) and T2 (after treatment), that is, a significant improvement in the intensity of symptoms of inattention, hyperactivity/impulsivity, ODD symptoms, behaviors of disobedience and its severity as well as the rule-breaking and aggressive behaviors. The waiting list group showed no significant difference between scores at T1 and T2. A higher rate of improvement in symptoms of inattention in children was predicted by the intensity of these symptoms before the intervention and attention difficulties of children in neuropsychological task. A higher rate of improvement in symptoms of hyperactivity in children was predicted by attentional difficulties of children in neuropsychological task. A higher rate of improvement in symptoms of ODD in children was predicted by the intensity of symptoms and difficulty of parents in cognitive flexibility task. Conclusion: The efficacy of treatment was confirmed in this sample. The worst performance of parents and children in neuropsychological tasks and a greater intensity of symptoms before Parents Training is associated with a higher rate of reduction in symptoms of inattention, hyperactivity and ODD in children
